HIV/AIDS患者外周血T淋巴细胞亚群与血清HIV RNA病毒载量的相关性研究

摘　　要：目的探讨HIV/AIDS患者外周血T淋巴细胞亚群与血清HIVRNA病毒载量的相关性。方法选取2017年1月-2018年6月在杭州市西溪医院就诊的艾滋病感染者40例以及AIDS患者32例,在未经抗病毒治疗前采用RT-PCR技术检测血清中HIVRNA载量,流式细胞仪进行T淋巴细胞亚群检测计数CD4^+、CD8^+T淋巴细胞。结果HIV感染组血清中HIVRNA检出率为75%(30/40),平均载量为(3.08±1.49) lgCopies/ml, CD4^+平均计数为(235.24土79. 53)个/μl, CD4^+/ CD8^+平均值为0.38±0.17;AIDS患者组血浆中HIV RNA检出率为87. 5%(28/32),平均载量为(4.57±2.25) lgCopies/ml,CD4^+平均计数为(135.24 ±55. 20)个/μl, CD4^+/ CD8^+平均值为0.23±0.14,两组间比,较差异有统计学意义(P<0.05);血清HIV RNA载量与CD4^+计数呈负相关(r=-0. 742,r=-0.561,P-<0.05)。结论AIDS患者较 HIV感染者HIV RNA平均载量高,CD4^+平均含量低;而CD4^+计数、CD4^+/ CD8^+比值与HIVRNA载量呈负相关,病毒载量越高,CD4^+计数、CD4^+/ CD8^+比值越低。Objective To investigate the correlation between peripheral blood T lymphocyte subsets and HIV RNA viral load among patients with HV/AIDS. Methods We totally recruited 40 HIV infected patients and 32 ADIS patients from January 2017 to June 2018 in Xixi Hospital of Hangzhou. The viral load was measured by the real - time fluorescence quantitative PCR (RT-PCR) and CD4^+,CD8^+ T cells were quantified using flow cytometry before their treatment. Results 75%( 30/40) HIV infected patients showed plasma HIV positive, the mean viral load was (3. 08±1. 49) lgCopies/ ml, the mean CD4^+ count was (235.24±79.53) cells/ul, CD4^+/CD8^+ ratio was 0.38 +0. 17;87.5%(28/32) AIDS patients had positive plasma RNA, viral load was (4. 57±2. 25) lgCopies/ml, CD4^+ count was ( 135. 24±55. 20) cells/ul, CD4^+/CD8^+ ratio was 0.23 +0. 14, and the difference was statistically significant between the infected patients and AIDS patients (P <0.05). Serum HIV RNA load was negatively correlated with CD4^+count (r=-0. 742,-0. 561, P <0.05). Conclusion Patients with AIDS had higher viral load and lower CD4^+counts compared with infected group. While CD4^+ count, CD4^+/CD8^+ were negatively correlated with HIV RNA viral load. The higher viral load, the lower CD4^+ count and CD4^+/CD8^+ ratio.
