肺结核患者血清高密度脂蛋白胆固醇、白细胞介素-6、高敏C-反应蛋白/白蛋白比值和降钙素原水平的临床研究

摘　　要：目的探讨活动性肺结核患者血清高密度脂蛋白胆固醇(HDL-C)、白细胞介素-6(IL-6)、高敏C-反应蛋白/白蛋白比值( hs-CRP/ALB)和降钙素原( PCT)水平的临床研究。方法95 例活动性肺结核进行了痰液结核分枝杆菌阳性率检查。采用化学发光免疫分析、免疫比浊法等分析了95例活动性肺结核和80例正常对照组血清HDL-C、IL-6、hs- CRP/ALB比值和PCT水平并进行了对比性研究。受试工作曲线( receiver operating characteristic curve ROC curve)评估活动性肺结核的临床价值。结果95例活动性肺结核患者痰液结核分枝杆菌阳性率为94.74%。95例活动性肺结核患者血清HDL-C、IL-6、hs-CRP/ALB比值和PCT水平较80例正常对照组明显增高,差异有统计学意义(P <0.05),血清HDL- C、IL-6、hs-CRP/ ALB比值和PCT临界值确定后,曲线下面积( area under curve, AUC)分别为0.814、0.913、0.845和0.871,均具有评估活动性肺结核的临床价值,并以血清IL-6为最佳。结论血清HDL-C、IL-6、hs-CRP/ALB比值和PCT是诊断活动性肺结核的有价值辅助诊断指标。Objective To explore clinical study serum HDL-C, IL-6,hs-CRP/ ALB ratio and PCT levels in active pulmonary tuberculosis. Methods In 95 patients with active pulumonary tuberculosis, the sputum positive rate was analyzed. The serum HDL-C, IL-6, hs- CRP/ALB ratio and PCT levels in 95 patients with active pulmonary tuberculosis and 80 controls were determined by chemiluminsence immunoassay and immunoturibidimetry for comparative study. The clinical value of active pulmonary tuberculosis was predicted by ROC Curve. Results The sputum positive rate in 95 patients with active pulumonary tuberculosis was 94. 74%. The serum HDL-C, IL-6, hs-CRP/ALB ratio and PCT levels obviously increased more than those of 80 controls ,with the differences statistically significant (P<0.05). T After the serum HDL-C, IL-6, hs-CRP/ ALB ratio and PCT threshold were determined, the area under the curve ( AUC) was0.814, 0.913, 0. 845 and 0.871, respectively, and the elinical value of evaluating active tuberculosis was evaluated. And serum IL- 6 was the best. Conclusion The serum HDL- C, IL-6,hs-CRP/ALB ratio and PCT levels were valuable assistant diagnosis index of active pulumonary tuberculosis.
