椎间孔镜和小切口开窗术式治疗腰椎间盘突出症的效果

摘　　要：目的分析椎间孔镜和小切口开窗术式治疗腰椎间盘突出症的效果。方法选取2016年9月至2018年7月我院收治的105例腰椎间盘突出症患者为研究对象,根据手术方式不同分为研究组(n=56)和对照组(n=49)。对照组接受经皮椎间孔镜髓核摘除术治疗,研究组接受小切口开窗髓核摘除术治疗。比较两组临床效果。结果研究组手术时间显著短于对照组(P<0.05);两组术中出血量、住院时间、术后下床活动时间比较,差异不显著(P>0.05)。术后1个月、半年,两组疼痛程度均显著改善(P<0.05)。术后半年,两组腰椎功能均显著改善(P<0.05)。结论经皮椎间孔镜髓核摘除术与小切口开窗髓核摘除术治疗腰椎间盘突出症患者的临床效果相近,但椎间孔镜术式对手术医生的要求较高,临床应综合考虑多种因素选择适宜的手术方式。Objective To analyze the effects of transforaminal endoscopic and small incision discectomy in the treatment of lumbar disc herniation. Methods A total of 105 patients with lumbar disc herniation admitted in our hospital from September 2016 to July 2018 were selected as the study objects. The patients were divided into study group (n=56) and control group (n=49) according to different surgical methods. The control group received percutaneous endoscopic lumbar discectomy, while the study group received small incision discectomy. The clinical effects of the two groups were compared. Results The operation time in the study group was significantly shorter than that in the control group (P<0.05);there were no significant difference in the amount of bleeding during operation, hospitalization time and the time of getting out of bed after operation between the two groups (P>0.05). One month and half a year after operation, the degree of pain in the two groups improved significantly (P<0.05). Half a year after operation, the lumbar function of the two groups improved significantly (P<0.05). Conclusion The clinical effect of percutaneous endoscopic lumbar discectomy and small incision discectomy in the treatment of lumbar disc herniation is similar, but transforaminal endoscopy is highly demanding for the surgeon, and many factors should be taken into account to select the appropriate surgical method.
