浸润性乳腺癌三维彩色能量多普勒成像血流参数值与Ki-67的相关性研究

摘　　要：目的:探讨浸润性乳腺癌三维彩色能量多普勒成像血流参数值与Ki-67表达的相关性。方法:分析105例经手术切除和病理确诊的浸润性乳腺癌肿块的三维超声血流信息和病理特征,采用QLab软件获得三维彩色能量多普勒血流参数值,包括血管指数(VI)、血流指数(FI)和血管血流指数(VFI),并与Ki-67的表达比较,分析相关性。结果:与Ki-67阴性组比较,Ki-67阳性组VI值和肿瘤最大直径更大,差异均有统计学意义(均P<0.05),且二者均与Ki-67的表达呈正相关(均P<0.05)。Ki-67的表达与乳腺癌组织学分级呈正相关(P<0.05)。Ki-67阳性更易出现淋巴结的转移(P<0.05)。VI值和VFI值在乳腺癌组织学分级和淋巴结分期中均有统计学差异(均P<0.05)。结论:浸润性乳腺癌三维彩色能量多普勒成像血流参数值与Ki-67表达存在一定的相关性,对评估浸润性乳腺癌的预后有一定价值。Objective:To investigate the correlation between blood flow parameter of three-dimensional color power doppler ultrasound and Ki-67 expression in invasive breast cancer.Methods:The blood flow information and pathological features of three-dimensional ultrasound were analyzed in 105 cases of invasive breast cancer resected by surgical and diagnosed by pathologic.Three-dimensional color power doppler blood flow parameters,including vascular index(VI),flow index(FI)and vascular flow index(VFI),were measured by QLab software.Then,the correlation between blood flow parameters mentioned above and Ki-67 expression in invasive breast cancer were analyzed.Results:Compared with the negative group,VI value and maximum tumor diameter in Ki-67 positive group were larger(Both P<0.05),and the two parameters were both positively correlated with the expression of Ki-67(Both P <0.05).The expression of Ki-67 was positively correlated with histological grade of breast cancer(P<0.05).The positive expression of Ki-67 was more prone to lymph node metastasis(P<0.05).Both in histology grades and lymph node stages,VI and VFI were performed with statistically difference,respectively(All P<0.05).Conclusion:There is a certain correlation between blood flow parameters of three-dimensional color power doppler ultrasound and Ki-67 expression in invasive breast cancer,which could be used for evaluating prognosis.
