摘　　要：目的探讨经尿道前列腺双极等离子电切术后尿白细胞阳性相关因素。方法选取2017年1月至2017年12月安徽省池州市人民医院诊治并行经尿道前列腺双极等离子电切术(PKPR)的63例患者资料作为研究对象。对影响术后尿白细胞阳性的相关因素进行分析。结果年龄、抗生素使用天数、术前导尿、前列腺重量与手术时间之比无统计学意义,与术后尿白细胞阳性无关。术前尿路感染(P=0.026)、术后尿管留置时间(P=0.025)、膀胱冲洗时间(P=0.008)与术后尿白细胞阳性有关。多因素logistic回归分析术前尿路感染、术后尿管留置时间、膀胱冲洗时间是术后尿白细胞阳性的相关因素。结论术前尿路感染、术后尿管留置时间为术后尿白细胞阳性的危险因素,膀胱冲洗时间为术后尿白细胞阳性的保护因素。术前积极控制感染、术后持续膀胱冲洗及早期拔除导尿管能降低术后尿白细胞阳性发生率。Objective To investigate the correlative factors of uroleukocytosis after transurethral resection of prostate(PKRP) with plasmakinetic energy. Methods The clinical data of 63 patients treated by PKRP from January 2017 to December 2017 were retrospectively analyzed, and the fators influencing uroleukocytosis were studied by univariate and multivariate analysis. Results The incidence of uroleukocytosis after PKRP was not related to age, days of antibiotic use, the ratio of prostate size to surgery time. The related factors included preoperative urinary tract infection(P=0.026), days of catheterization after surgery(P=0.025) and days of bladder irrigation(P=0.008). The multivariate logistic regression analysis showed that the preoperative urinary tract infection, days of catheterization after surgery, days of bladder irrigation were the independent related factors of uroleukocytosis after PKRP. Conclusions The preoperative urinary tract infection and days of catheterization after surgery are the risk factors of uroleukocytosis after PKPR, and days of bladder irrigation is a protective factor of uroleukocytosis after PKPR. Therefore, it is necessary to positively control infection before operation, keep bladder irrigation continued and shorten the duration of catheter indwellingafter surgery, in order to reduce the incidence of uroleukocytosis after PKPR.
