摘　　要：目的探讨内镜下十二指肠乳头小切开(small endoscopic sphincterotomy,SEST)联合胆道球囊扩张治疗老年患者胆管结石的疗效和安全性。方法回顾性分析石河子大学第三附属医院消化内科2014至2016年完成的十二指肠镜下逆行胰胆管造影(endoscopic retrograde cholangiopancreatography,ERCP)的老年胆管结石住院患者临床资料。结果57例老年胆管结石患者行ERCP术,插管造影成功55例(96%)完成治疗。其中46例完成SEST+胆道球囊扩张,一次性取尽结石率达83.6%。术前肝功、血尿淀粉酶与术后比较差异均有统计学意义(P<0.05)。结论SEST联合胆道球囊扩张治疗老年患者胆管结石效果肯定,安全性好。Objective To study the curative effect and safety of small endoscopic sphincterotomy (SEST) combined with biliary balloon dilatation by endoscopy on bile duct calculus in old patients.Methods Old patients with bile duct calculus were administered with endoscopic retrograde cholangiopancreatography (ERCP) in Gastroenterology Department of Third Affiliated Hospital of Shihezi University from 2014 to 2016,and the clinical data of whom were retrospectively analyzed.Results ERCP was performed in 57 old patients with bile duct calculus,and 55 cases (96%) were successfully treated by cholangiography.Among them,46 cases were treated by SEST and biliary balloon dilatation.The one-time stone removal rate was 83.6%.The changes of liver function,blood amylase,urine amylase before and after operation were observed ( P <0.05).Conclusion SEST combined with biliary balloon dilatation by endoscopy is a safe and effective treatment method for bile duct calculus in old patients.
