摘　　要：目的探讨颗粒蛋白前体(PGRN)在皮肌炎(DM)合并间质性肺疾病(ILD)患者的表达及临床意义。方法DM患者50例,22例合并ILD(A组),28例未合并ILD(B组);选取健康体检者50例作为对照(C组)。A、B组均采用糖皮质激素治疗,A组在此基础上加用免疫抑制剂,治疗3个月。检测A、B组治疗前及C组血清PGRN、CRP、铁蛋白、涎液化糖链抗原(KL-6)、TNF-α水平,并分析PGRN与CRP、铁蛋白、KL-6、TNF-α的相关性。检测A组治疗后血清PGRN、TNF-α水平。结果A、B组患者治疗前血清PGRN、CRP、铁蛋白、KL-6、TNF-α水平均高于C组(P<0.05)。A组治疗前血清PGRN、CRP、铁蛋白、KL-6水平较B组升高(P<0.05)。A组治疗前血清PGRN与铁蛋白、KL-6、CRP及TNF-α呈正相关(P<0.05)。A组治疗后血清PGRN、TNF-α水平较治疗前下降(P<0.05)。结论DM合并ILD患者血清PGRN水平升高,与DM疾病活动和ILD的发生有关。Objective To investigate the expression and clinical significance of progranulin (PGRN)in the patients with dermatomyositis(DM)combined with interstitial lung disease(ILD). Methods The patients with DM were divided into groups of A (combined with ILD,22 cases)and B (without ILD,28 cases).The healthy people were selected as the controls(group C,50 cases).The patients in groups of A and B were treated with glucocorticoid and the patients in group A were additionally treated with immunosuppressant for 3 months.Serum levels of PGRN,CRP,ferritin, Krebs von den Lungen-6(KL-6)and TNF-αwere measured in groups of A and B before treatment and in group C.The relationship of PGRN and CRP,ferritin,KL-6 and TNF-α was analyzed.The levels of PGRN and TNF-α were detected in group A after treatment.Results Serum levels of PGRN,CRP,ferritin,KL-6 and TNF-α were higher in groups of A and B before treatment than those in group C(P<0.05).Serum levels of PGRN,CRP,ferritin and KL-6were higher in group A than those in group B before treatment(P<0.05).Serum level of PGRN was positively correlated to serum levels of ferritin,KL-6,CRP and TNF-α in group A before treatment(P<0.05).Serum levels of PGRN and TNF-α were lower after treatment than those before in group A(P<0.05).Conclusion Serum PGRN level is higher in the patients with DM combined with ILD,which may be related to disease activity and the occurrence of ILD.
