摘 要:目的探讨多模态融合骨显像对肺癌骨转移诊断的增益价值。方法112例肺癌患者,静脉注射99Tc^m-亚甲基二磷酸盐925MBq后行单光子发射计算机断层显像(SPECT)全身骨显像和SPECT/CT多模态融合骨显像。比较全身骨显像和多模态融合骨显像诊断肺癌骨转移的灵敏度、特异度及准确度。结果112例肺癌患者中,骨转移者65例,非骨转移者47例。多模态融合骨显像诊断肺癌骨转移的灵敏度、特异度和准确度均优于全身骨显像(93.8%vs.70.8%、95.7%vs.68.1%和94.6%vs.69.6%)(χ^2=11.89、12.14和23.86)(P<0.05)。结论多模态融合骨显像实现功能显像和解剖影像的有效结合,对肺癌患者骨转移诊断具有较高增益价值。Objective To investigate the gain value of multimodal fusion bone imaging in the diagnosis of bone metastasis of lung cancer.Methods A total of 112 patients with lung cancer was given an intravenous injection of 99Tc^m-methylene diphosphonate 925 MBq,single photon emission computed tomography(SPECT)total body bone imaging and multi-modal fusion bone imaging were performed.The sensitivity,specificity and accuracy for the two techniques to diagnose bone metastasis of lung cancer were compared.Results Sixty five cases were comfirmed to have bone metastasis and 47 cases were not.The sensitivity,specificity and accuracy of multi-modal fusion bone imaging in the diagnosis of bone metastasis of lung cancer were better than those of whole body bone imaging (93.8%vs.70.8%,95.7% vs.68.1% and 94.6% vs.69.6%)(χ^2 =11.89,12.14and 23.86, P<0.05).Conclusion Multimodal fusion bone imaging can effectively combine functional imaging with anatomical imaging,which has a higher gain value in the diagnosis of bone metastasis of lung cancer.
