摘　　要：目的对2013—2018年武汉市焦炉逸散物所致肺癌的诊断病例进行分析,为焦炉逸散物所致肺癌的预防工作及控制措施提供依据。方法根据武汉市2013—2018年诊断上报的焦炉逸散物所致肺癌病例的临床资料、职业史、工作环境等因素,分析焦炉逸散物所致肺癌的发病情况并进行数据分析。结果焦炉逸散物所致肺癌患者多在退休后发病,工龄越长发病率越高,发病高峰年龄为67岁,潜隐期多为44年左右。最常见的发病工种是炉顶工作的炉盖工和出炉工,因为焦炉逸散物在炉顶浓度最高。病理检查显示主要是鳞癌和腺癌,腺癌多于鳞癌。结论需加强焦炉工人的职业防护以预防焦炉逸散物所致肺癌,对焦炉工人建立健康监护档案并定期复查,特别是退休后的焦炉工人,做到早发现早治疗。Objective To analyze the diagnosis cases of lung cancers caused by coke oven emissions in Wuhan from 2013 to 2018,and to provide a basis for prevention and control of lung cancers caused by coke oven emissions.Methods According to the clinical data,occupational history and working environment of lung cancer cases caused by coke oven emissions reported in Wuhan from 2013 to 2018,the incidence of lung cancers caused by coke oven emissions was analyzed and data analysis was carried out.Results Most of the lung cancer patients caused by coke oven emissions occurred after retirement.The longer the service age was,the higher the incidence was.The peak age of incidence was 67 years,and the latent period was about 44 years.The most common types of work were the covers and tappers working on the top of the furnace,because the concentration of coke oven emissions was the highest on the top of the oven.Pathological examination showed that squamous cell carcinoma and adenocarcinoma were the main types,and adenocarcinoma was more than squamous cell carcinoma.Conclusion It is necessary to strengthen occupational protection of coke oven workers to prevent lung cancers caused by coke oven emissions,and to establish health surveillance files for coke oven workers and to review them regularly,especially for retired coke oven workers,to achieve early detection and early treatment.
