摘 要:目的分析酶联免疫斑点技术诊断肺结核病的效果,评价该技术在肺结核辅助诊断中的应用价值。方法收集428例疑似肺结核患者的血液及痰液,分别用ELISPOT技术和传统分离培养法检测结核分枝杆菌。以临床肺结核诊断为标准,比较上述两种检测方法应用价值。结果ELISPOT对428例研究对象的阳性检出率49.07%,分离培养法阳性检出率18.69%,二者检出率差异有统计学意义(χ^2=20.6,P<0.05);两种方法检测效果差异有统计学意义(χ^2=15.34,P<0.05),ELISPOT检测方法检测时限、操作简便性优于分离培养方法。结论ELISPOT检测方法在肺结核辅助诊断中敏感度和特异性较高,检测周期明显短于传统分离培养法,对疑似结核患者的诊断具有重要参考价值。Objecve To analyze the effect of enzyme-linked immunosorbent assay(ELISPOT)in the diagnosis of pulmonary tuberculosis and to evaluate the value of ELISPOT in the clinical diagnosis of pulmonary tuberculosis.Methods Blood and sputum of 428 patients with suspected pulmonary tuberculosis were collected.ELISPOT technique and traditional isolation culture method were used to test mycobacterium tuberculosis.Based on the clinical diagnosis of tuberculosis,the application value of the above two methods in the diagnosis of pulmonary tuberculosis was compared.Results The positive detection rate of ELISPOT in 428 subjects was 49.07%,and the positive detection rate of isolation and culture method was 18.69%.There was significant difference in the detection rate between the two methods(χ^2=20.6,P<0.05).The sensitivity and specificity of ELISPOT detection method were 79.81%and 81.40%,and the positive predictive and negative values were 80.95%and 80.28%,respectively.The Youden index was 0.612.The sensitivity and specificity of isolation and culture method were 37.56%and 100%,and the positive predictive and negative values were 100%and 61.78%.The Youden index was 0.376.The detection effect of the two methods was significantly different(χ^2=15.34,P<0.05).ELISPOT detection method was better than the traditional separation culture method in terms of the detection timing and operational convenience.Conclusion ELISPOT method had higher sensitivity and specificity,and shorter detection time than traditional isolation and culture method in auxiliary diagnosis of pulmonary tuberculosis.The ELISPOT method has important reference value for the clinical diagnosis of suspected tuberculosis.
