对腹膜透析患者实施一体化管理的效果评价

摘　　要：目的探讨专业医护人员组成专职团队对腹膜透析(peritoneal dialysis,PD)患者实施一体化管理的效果。方法选择2009年6月-2012年5月于四川省宜宾市第二人民医院血液净化中心接受维持性PD治疗的226例患者实施一体化管理模式,观察患者的实验室指标、护理指标达标率、心脏影像学在实施1年内的变化情况,并统计腹膜炎发生情况。结果与出院当天相比,本组患者在PD治疗3个月、6个月、12个月时Alb、TP水平、体重达标率、血压达标率、自我管理能力达标率和随访达标率逐渐上升,血磷水平逐渐降低,不同时间点差异有统计学意义(P<0.05)。腹膜炎发生情况统计结果显示,2010-2012年的发生率分别为1/37.7、1/56.5、1/75.3次患者月,发生率整体呈下降趋势。与出院当天相比,本组患者在PD治疗1年期间心胸比、左心房内径、右心室内径、室间隔厚度均无显著变化(P>0.05)。与出院当天相比,本组患者在PD治疗1年期间自护技能、自护责任感、自我概念、健康知识、总分均逐渐上升,不同时间点差异有统计学意义(P<0.05)。结论将一体化管理模式应用于PD患者的护理中,能有效改善PD患者的生理健康状况,提高患者的自我效能,具有重要的临床应用价值。Objective To explore the application value of integrated management model in peritoneal dialysis patients. Methods From June 2009 to May 2012, a total of 226 chronic renal failure patients with peritoneal dialysis in our hospital's blood purify center were applied integrated nursing model. Laboratory parameters, care indicators compliance rate, cardiac imaging were observed. Results Compared to the day of discharge, the level of Alb and TP, weight compliance rate, rate of blood pressure, self-management and follow-up compliance rate compliance rate rose gradually at PD treatment for 3 months, 6 months, 12 months, and the phosphorus levels decreased (P<0.05). Peritonitis occurrence statistics showed that from 2010 to 2012, the incidence rate of 1/37.7, 1/56.5, 1/75.3 times a month in patients, the incidence of the overall downward trend. Compared to the day of discharge, the cardiothoracic ratio, left atrium, right ventricular diameter, interventricular septum thickness did not change significantly (P>0.05). Compared to the day of discharge, the self-care responsibility, self-concept, health knowledge scores were gradually increased significantly (P<0.05). Conclusion Integrated management model for the treatment of chronic renal failure have a significant effect, improving the quality of life of patients, and deserves further study in clinical practice.
