摘 要:目的探讨雾霾对小儿哮喘患者肺功能和调节性T细胞亚群的影响。方法本研究分别选取来自无雾霾地区(无雾霾组)、轻度雾霾污染地区(轻度雾霾组)以及重度雾霾污染地区(重度雾霾组)的儿童为研究对象,每组30例。比较3组患儿肺功能及调节性T细胞亚群水平。结果重度、轻度雾霾组受试者FEV1、FVC以及FEV1/FVC值均明显低于无雾霾组,重度雾霾组受试者上述肺功能指标均明显低于轻度雾霾组,3组间差异均有统计学意义(P<0.05)。重度、轻度雾霾组受试者CD3^+、CD4^+水平以及CD4^+/CD8^+值均明显低于无雾霾组,CD8^+水平明显高于无雾霾组,差异均有统计学意义(P<0.05);重度雾霾组受试者CD3^+、CD4^+水平以及CD4^+/CD8^+值均明显低于轻度雾霾组,CD8^+水平明显高于轻度雾霾组,差异具有统计学意义(P<0.05)。重度、轻度雾霾组受试者IL-4、IL-5水平以及IL-4/INF-γ值明显高于无雾霾组,INF-γ水平明显低于无雾霾组,差异均有统计学意义(P<0.05);重度雾霾组受试者IL-4、IL-5水平以及IL-4/INF-γ值明显高于轻度雾霾组,INF-γ水平明显低于轻度雾霾组,差异均有统计学意义(P<0.05)。重度、轻度雾霾组受试者痰液中PDGF、TGF-β1、c-fos mRNA相对表达值均明显高于无雾霾组,重度雾霾组受试者PDGF、TGF-β1、c-fos mRNA相对表达值明显高于轻度雾霾组,差异均有统计学意义(P<0.05)。结论雾霾中的细颗粒物进入小儿哮喘患者呼吸道内可增加其呼吸道阻力,引起患儿肺功能下降,并通过影响机体调节性T细胞亚群水平,降低患儿免疫功能。Objective To investigate the effects of haze on lung function and regulatory T cell subsets in children with asthma.Methods In this study,the children were selected from no haze area(haze free group,n=30),mild haze pollution area(mild haze group,n=30)and severe haze pollution area(severe haze group,n=30)to compare the lung function and regulatory T cell subgroup among the three groups.Results The levels of FEV1,FVC and FEV1/FVC in severe and mild haze group were significantly lower than those in haze free group,and those in heavy haze group significantly lower than those in mild haze group(P<0.05).The levels of CD3^+,CD4^+and CD4^+/CD8^+in severe and mild haze groups were significantly lower than those in haze free group,but the levels of CD8^+were significantly higher than those in haze free group(P<0.05).The levels of CD3^+,CD4^+and CD4^+/CD8^+value in severe haze group were significantly lower than those in mild haze group,however,the levels of CD8^+were significantly higher than those in mild haze group(P<0.05).The levels of IL-4,IL-5 and IL-4/INF-γin severe and mild haze groups were significantly higher than those in haze free group,and the levels of INF-γwere significantly lower than those in haze free group(P<0.05).The levels of IL-4,IL-5 and IL-4/INF-γvalue in severe haze group were significantly higher than those in mild haze group,and the levels of INF-γwere significantly lower than those in mild haze group(P<0.05).The relative expression values of PDGF,TGF-β1 and c-fos mRNA in severe and mild haze group were significantly higher than those in haze free group,and those in severe haze group were significantly higher than those in mild haze group(P<0.05).Conclusion The fine particles in haze can increase air way resistance in children with asthma,cause the decline of the lung function,and reduce the immune function of patients by affecting the levels of regulatory T cell subgroup.
