四种癌症患者长期治疗费用及影响因素分析——以济南市某三甲医院为例

摘　　要：目的:分析四种癌症患者的长期治疗费用及影响因素,为减轻患者的疾病经济负担提供参考依据。方法:以2011~2014年在济南市某三甲医院首次确诊为肠癌、肺癌、乳腺癌和胃癌的住院患者为研究对象。描述患者的人口特征、住院情况和治疗费用,运用广义线性回归模型对长期治疗费用的影响因素进行分析。结果:四种癌症患者的人均长期治疗费用分别为肺癌(115344.59元)、肠癌(114965.93元)、胃癌(105571.12元)和乳腺癌(92905.33元);自付费用分别为38772.21元、37992.71元、30120.57元、36972.65元。长期治疗费用的主要影响因素有住院天数、医保类型、是否手术等。结论:癌症患者疾病经济负担沉重,建议推进癌症早期筛查、提高医保报销水平、规范临床路径。Objective To analyze the long-term treatment expenses and its main influencing factors of 4 kinds of patients with common cancers, and provide references for reducing the medical expenses of cancer patients. Methods Patients with first diagnosis of colorectal cancer, lung cancer, breast cancer, and stomach cancer in a tertiary hospital in Jinan from 2011 to 2014 were selected. The characteristics, hospitalization and treatment expenses were descriptively explained. The generalized linear model was used to examine the influencing factors of treatment expenses. Results The average long-term treatment expenses of lung, colorectal, stomach, and breast cancer were respectively 115 344.59 yuan, 114 965.93 yuan, 105 571.12 yuan and 92 905.33 yuan. The out-of-pocket expenses of lung, colorectal, stomach, and breast cancer were respectively 38 772.21 yuan, 37 992.71 yuan, 30 120.57 yuan and 36 972.65 yuan. The major influencing factors of treatment expenses were insurance type, receipt of surgery, and length of stay. Conclusion The economic burden of cancer patients was heavy. The cancer prevention and control should be promoted, the reimbursement should be improved, and clinical pathway should be standardized.
