摘　　要：目的:为慢性肾脏病患者的药物治疗提供依据。方法:临床药师参与慢性肾脏病5期(CKD5期)伴肾性骨病及肾性贫血患者的药物治疗过程及监护,探讨在治疗肾性骨病过程中的药物选择及对两种常用药物进行分析比较,对临床精准用药提供参考。结果:患者病情稳定,钙、磷水平达标。结论:临床药师深入临床,进行药学服务,有利于提高临床药物治疗水平。Objective:To provide a reference for drug therapy for chronic kidney disease. Methods:Clinical Pharmacists Participated in Drug Treatment course and Supervision for a Patient with Chronic Kidney Disease Phase 5 (CKD5) Complicated with Renal Osteopathy and Renal Anemia, To explore the choice of drugs in the treatment of renal osteopathy and to compare the two commonly used drugs in order to provide reference for clinical precise medication.Results:The conditions of the patient was stable , the calcium and phosphorus level is up to standard. Conclusion:involvement of clinical pharmacists in the therapeutic practice can improve the level of drug therapy.
