摘 要:目的观察无创高频通气(NHFV)应用于新生儿急性呼吸窘迫综合征(NRDS)撤机后呼吸支持的效果。方法按照随机数字表法将在我院行有创高频震荡通气的42例NRDS患儿分为实验组和对照组,各21例。对照组拔管后采用经鼻间歇正压通气(NIPPV)辅助通气,实验组拔管后采用NHFV辅助通气。比较两组患儿的撤机成功率、呼吸暂停发生例次、二氧化碳分压(PaCO2)水平及并发症发生率。结果实验组撤机成功率高于对照组,呼吸暂停发生例次及PaCO2水平低于对照组,差异均有统计学意义(P<0.05);两组患儿并发症发生率相比较,差异无统计学意义(P>0.05)。结论NHFV作为NRDS患儿撤机后提供的呼吸支持模式效果良好,有利于提高患儿撤机成功率,减少治疗过程中呼吸暂停例次,有效控制PaCO2水平,减少并发症的发生。Objective To observe the effect of non-invasive high-frequency ventilation (NHFV) on respiratory support for neonatal acute respiratory distress syndrome (NRDS) after removal of ventilator. Methods According to the random number table method,42 children with NRDS who underwent high-frequency oscillatory ventilation in our hospital were divided into experimental group and control group,with 21 cases in each group.After the extubation,the control group was given nasal ventilation plus positive ventilation (NIPPV) assisted ventilation while the experimental group was given NHFV assisted ventilation.The success rate of weaning,the cases of apneas,PaCO2 level and the incidence of complications were compared between the two groups. Results The success rate of weaning in the experimental group was higher than that in the control group,and cases of apnea and PaCO2 level of the experimental group were lower than those of the control group.The difference was statistically significant (P < 0.05).There was no statistically significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion NHFV as a respiratory support model provided for children with NRDS is effective,which is beneficial to improve the success rate of weaning,reduce the cases of apnea during treatment,effectively control PaCO2 level and reduce the occurrence of complications.
