单纯全宫切除与全宫切除术中同时切除双侧输卵管对绝经前女性卵巢功能的不同影响

摘　　要：目的探讨单纯全宫切除与全宫切除术中同时切除双侧输卵管对绝经前女性卵巢功能的影响。方法选取本院2017年6月~2018年6月间200例全子宫切除及双侧输卵管切除患者进行研究,均为绝经前女性,采用随机数字表法分为实验组与对照组,各100例。对照组行全子宫切除术,实验组行全子宫切除并双侧输卵管切除,观察两组术前、术后2个月、术后1年抗苗勒管激素(AMH)水平变化及术后并发症情况。结果两组术后2个月、1年AMH水平较术前逐步均降低,术前术后比较,差异有统计学意义(P<0.05);两组间在术后2个月、术后1年AMH水平上,差异无统计学意义(P>0.05)。实验组输卵管病变、盆腔包块发生率显著低于对照组,差异有统计学意义(P<0.05)。结论单纯全子宫切除与全宫切除同时切除双侧输卵管均会对绝经前女性卵储备功能产生影响,全宫切除同时切除双侧输卵管并不会加重这种影响,并能减少术后并发症,可依据患者实际情况及意愿选择适合的治疗方式。Objective To explore the effects of simple total hysterectomy and bilateral fallopian tubes excision in total hysterectomy on ovarian function in premenopausal women. Methods 200 premenopausal women who underwent total hysterectomy and bilateral fallopian tubes excision in our hospital from June 2017 to June 2018 were selected as research objects. According to the random number table method, they were divided into the experimental group and the control group, with 100 cases in each group. The control group was given total hysterectomy while the experimental group was given total hysterectomy and bilateral fallopian tubes excision. The changes of anti-muller hormone (AMH) levels and postoperative complications were observed before operation, 2 months and 1 year after operation in two groups. Results AMH levels of 2 months and 1 year after operation of two groups were gradually lower than those before operation. The difference was statistically significant (P < 0.05). There was no statistically significant difference in AMH level of 2 months and 1 year after surgery between the two groups (P > 0.05). The incidence of fallopian tube lesions and pelvic mass in the experimental group was significantly lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion The simple total hysterectomy and bilateral fallopian tubes excision in total hysterectomy both have effects on ova reserve function of premenopausal females. Bilateral fallopian tubes excision in total hysterectomy does not aggravate this effect and it can reduce postoperative complications. Suitable treatment methods can be selected according to patients’ actual conditions and wishes.
