热休克蛋白70检测在急性心肌梗死诊断及治疗中的意义

摘　　要：目的探讨热休克蛋白70(HSP70)检测在急性心肌梗死(AMI)诊断及治疗中的意义。方法选择住院的AMI患者160例为试验组;选择同期在本院正常体检者40例为对照组。试验组分别于急性期和恢复期观察各指标。采用酶联免疫分析法(双抗体夹心法)检测血清HSP70水平,比较2组血清HSP70、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、肌钙蛋白T(cTnT)、B型钠尿肽(BNP)、肌红蛋白(MYO);采用Spearman相关性分析,logistic回归分析及ROC曲线分析HSP70对AMI诊断价值。结果试验组急性期血清HSP70明显高于对照组和同组恢复期[25.37(10.32,50.08)μg/L vs 0.088(0.215,0.557)μg/L和0.092(0.027,0.976)μg/L,P<0.01]。试验组HSP70的ROC曲线下面积为0.722(95%CI:0.982~0.999),CK 0.883(95%CI:0.837~0.929)、CK-MB 0.929(95%CI:0.894~0.964)、cTnT 0.949(95%CI:0.918~0.980)、BNP 0.853(95%CI:0.923~0.982)、MYO 0.775(95%CI:0.705~0.845)。血清HSP70与CK、CK-MB、cTnT、MYO呈正相关(P=0.000)。logistic回归分析显示,HSP70与AMI发病独立相关(OR=3.201,95%CI:0.248~0.414)。结论 AMI患者血清HSP70明显增高,血清HSP70可能可以作为AMI的早期诊断指标。Objective To study the significance of HSP70 in diagnosis and treatment of AMI. Methods One hundred and sixty AMI patients admitted to our hospital served as an experimental group and 40 persons who underwent physical examination in our hospital served as a control group. The clinical data of experimental group were recorded during the acute and recovery periods. The significance of HSP70 in diagnosis of AMI was analyzed by logistic regression analysis and ROC curve analysis respectively. Results The serum HSP70 level was significantly higher in experimental group than in control group [25.37(10.32,50.08)μg/L vs 0.088(0.215,0.557)μg/L,0.092 (0.0273,0.976)μg/L, P<0.01].The area under the ROC curve for HSP70, CK, CK-MB,cTnT,BNP and MYO was 0.722 (95% CI:0.982-0.999),0.883 (95%CI:0.837-0.929),0.929 (95% CI:0.894-0.964),0.949 (95% CI:0.918-0.980),0.853 (95% CI:0.923-0.982),0.775(95% CI:0.705-0.845) respec-tively. Spearman correlation analysis showed that the serum levels of HSP70 were positively related with those of CK, CK-MB, cTnT and MYO (r= 0.38,r=0.32,r=0.40,r=0. 54,P = 0.000). Logistic regression analysis showed that HSP70 was independently related with AMI (OR = 3.201,95% CI:0.248-0.414). Conclusion Elevated serum HSP70 level can be used as a biomarker for the early diagnosis of AMI.
