静态进展型矫形器对Ⅱ区手指屈肌腱断裂术后肌腱挛缩的疗效分析

摘　　要：目的:探讨静态进展型矫形器对Ⅱ区手指屈肌腱断裂术后肌腱挛缩的矫治效果。方法:52例Ⅱ区手指屈肌腱断裂术后6周患者,随机分为两组。试验组采用常规康复治疗联合静态进展型矫形器矫治,对照组仅采用常规康复治疗,治疗持续4周。分别在治疗前、治疗2周、治疗4周后评定伤指总主动活动度(TAM)、总被动关节活动度(TPM)和简易上肢功能评价量表(STEF)。结果:经过2周和4周治疗后,两组患者伤指TAM、TPM及STEF评分均有明显改善(P<0.05),且试验组的伤指TAM、TPM及STEF评分在治疗2周后和治疗4周后均分别显著优于对照组(P<0.05)。结论:静态进展型矫形器能够提高伤指运动幅度及伤指运动功能,可能与屈肌腱弹性和延展度改善有关,是Ⅱ区手指屈肌腱断裂术后肌腱挛缩的有效矫治方法。Objective:The purpose of this study is to investigate the effect of static progressive splinting on tendon contracture after operating on rupture of finger flexor tendon in zone Ⅱ. Method:Fifty-two cases after operating on rupture of finger flexor tendon in zone Ⅱ in 6 weeks were randomly divided into two groups. The experimental group received routine rehabilitation therapy and static progressive splinting, while the control group received conventional rehabilitation only. The treatment lasted for 4 weeks. Before treatment, 2 and 4 weeks after treatment, total active range of motion (TAM) and total passive range of motion (TPM) of injury fingers, and the simple test for evaluating hand function(STEF) were measured. Result:The scores of TAM, TPM, and STEF of the injury fingers in both groups were significantly improved after treatment for 2 and 4 weeks (P<0.05). Besides, they are improved more in the experimental group than those in control group after 2 and 4 weeks treatment (P<0.05). Conclusion:Static progressive splinting can increase the range of motion and the motor function of fingers, which may be related to the improvement of flexor tendon elasticity and extension, and it is an effective method for tendon contracture after operation on rupture of finger flexor tendon in zone Ⅱ.
