心房颤动对踝肱指数测量的影响

摘　　要：目的研究心房颤动是否会影响踝肱指数(ankle-brachial index,AB)的测量结果。方法纳入了2016年7月至2018年9月在东莞市人民医院心内科住院的阵发性心房颤动患者138例(其中男63例、女75例),在心房颤动发作前后或心房颤动终止前后分别进行ABI测定。每次ABI测定均测定3次后求均值。结果患者心房颤动时测得的ABI低于窦性心率下测得的ABI(左:1.113±0.073vs.1.147±0.069,P<0.001;右:1.114±0.061vs.1.149±0.058,P<0.001);在快心房颤动组中该差异更加明显(左:1.105±0.076vs.1.146±0.074,P<0.001;右:1.108±0.062vs.1.149±0.061,P<0.001);然而在慢心房颤动组中,该差异无明显统计学意义(左:1.151±0.044vs.1.154±0.043,P=0.070;右:1.144±0.040vs.1.148±0.043,P=0.095)。结论心房颤动对ABI测量的主要影响在于心房颤动时的心室率,快心室率时会降低ABI的测量结果。Objectives To investigate whether atrial fibrillation would affect the outcome of ankle-brachial index (ABI).Methods This study included 138 patients(including 63 males and 75 women)with paroxysmal atrial fibrillation admitted to Dongguan People′s Hospital from July 2016 to September 2018.ABI measurements were performed before and after the onset of atrial fibrillation or before and after the termination of atrial fibrillation.The mean value was determined after each ABI measurement was measured three times.Results ABI measured in patients with atrial fibrillation was lower than that in patients with sinus rhythm(left:1.113±0.073 vs.1.147±0.069,P<0.001;right: 1.114±0.061 vs.1.149±0.058,P<0.001);the difference was significant in rapid atrial fibrillation subgroup(left: 1.105±0.076 vs.1.146±0.074,P<0.001;right:1.108±0.062 vs.1.149±0.061,P<0.001),whereas in slow atrial fibrillation subgroup,the difference was not significant with statistical differences(left:1.151±0.044 vs.1.154±0.043,P= 0.070;right:1.144±0.040 vs.1.148±0.043,P=0.095).Conclusions The main influence of atrial fibrillation on ABI measurement lies in the heart rate during atrial fibrillation:faster heart rate can reduce the measurement results of ABI.
