柴胡软肝散治疗乙肝早期肝硬化(肝脾不调型)的临床研究

摘　　要：目的使用瞬时弹性成像联合血清学肝纤维化标志物检测的方法,评价在恩替卡韦抗病毒治疗基础上,柴胡软肝散治疗乙肝早期肝硬化(肝脾不调型)的临床疗效。方法60例乙肝早期肝硬化(肝脾不调型)患者分为治疗组和对照组,30例/组,治疗组予恩替卡韦片0.5 mg,QD,同时使用中药柴胡软肝散。对照组给与恩替卡韦片0.5 mg,QD。连续用药6个月对比疗效结果。结果治疗6个月,治疗组FS(10.75±3.79)k Pa低于对照组FS(15.43±4.18)k Pa,治疗组HA(108.5±32.3)ng/m L低于对照组HA(160.9±31.1)ng/m L,治疗组ALT(21.34±4.81)IU/L低于对照组ALT(30.35±2.92)IU/L,治疗组AST(19.59±3.16)IU/L低于对照组AST(26.52±3.32)IU/L,2组间有显著差异(P<0.05)。治疗组治疗有效率为90.00%,高于对照组的66.67%,差异具有统计学意义(P<0.05)。结论在恩替卡韦抗病毒治疗基础上联合柴胡软肝散治疗乙肝早期肝硬化(肝脾不调型)效果显著,能够有效改善患者FS、HA以及肝功能水平,抑制乙肝早期肝硬化发展。Objective To evaluate the clinical efficacy of chaihu ruangan powder in the treatment of early hepatitis B cirrhosis (hepatosplenic dysregulation) on the basis of entecavir antiviral therapy by using instantaneous elastography combined with serological detection of liver fibrosis markers. Methods 60 patients with early cirrhosis of hepatitis B were divided into treatment group and control group, 30 patients in each group. The control group was given entecavir tablets 0.5 mg, QD. The results of 6 months continuous medication were compard. Results After 6 months of treatment, FS (10.75±3.79) kPa in the treatment group was lower than that in the control group (15.43± 4.18) kPa, HA (108.5 ± 3 2 . 3) ng/mL in the treatment group was lower than that in the control group (160.9±31.1) ng/mL, ALT (21.34±4.81) IU/L in the treatment group was lower than that in the control group (30.35±2.92) IU/L, and AST (19.59±3.16) IU/L in the treatment group was lower than that in the control group (26.5 2 ± 3 . 32) IU/L. The effective rate was 90.00% in the treatment group and 66.67% in the control group, which was statistically significant (P <0.05). Conclusion On the basis of entecavir antiviral therapy, combined with Chaihu Ruangan Powder, the treatment of early hepatitis B cirrhosis (disharmony between liver and spleen type) has a significant effect, which can effectively improve the level of FS, HA and liver function of patients, and inhibit the development of early hepatitis B cirrhosis.
