摘 要:目的探讨病灶-肺组织造影剂到达时间差在判断富血供肺周围型病变性质的应用价值。方法选取80例富血供肺周围型病变患者,均行穿刺活检或手术病理检查,根据病理结果分为恶性肿瘤病变组和炎性病变组。所有患者均行CEUS,结束后应用时间-强度曲线定量分析软件分析病灶、邻近肺组织造影剂到达时间(AT),计算出病灶-肺组织造影剂时间差,绘制受试者工作特征曲线(ROC),计算曲线下面积及最佳时间截断点。结果80例富血供型肺周围型病变中炎性病变33例,恶性肿瘤性病变47例。恶性肿瘤病变组病灶-肺组织造影剂AT差为(4.12±2.10)s,炎性病变组为(1.16±1.47)s,二者比较差异有统计学意义(P<0.05)。ROC曲线显示病灶造影剂AT、病灶-肺组织造影剂到达时间差曲线下面积分别为0.762、0.883,由Youden指数确定最佳时间截断点分别为6.5、2.4s,其灵敏度分别为72.9%、85.4%,特异度分别为71.9%、87.5%。结论病灶-肺组织造影剂到达时间差在肺周围型病变诊断中的灵敏度、特异度均优于病灶造影剂AT,对良恶性的鉴别诊断具有一定的应用价值。Objective To investigate the diagnostic value of the arrival time difference between lesions and its adjacent gas-containing lung tissue by contrast-enhanced ultrasound in peripheral lung lesions with abundant blood supply.Methods 80 peripheral lung lesions with abundant blood supply were selected for biopsy or surgical pathological examination.They were divided into malignant tumor group and inflammatory disease group according to pathological results.The arrival time(AT)of contrast agent in the peripheral lung lesion and adjacent lung tissue and the time difference between them were calculated by time-intensity curve(TIC).The receiver operating characteristic curve(ROC)was plotted to calculate the area,under the curve and the optimum threshold.Results There are 33 inflammatory lesions and 47 malignant tumor lesions in 80 peripheral pulmonary lesions with abundant blood supply.The lesion-lung time difference was(4.12±2.10)seconds in the malignant lesion group,and(1.16±1.47)seconds in the inflammatory lesion group,and the difference was statistically significant(P<0.05)between them.The ROC curve showed that the area under the curve of arrival time of contrast agent in the peripheral lung lesion and the lesion-lung time difference were 0.762 and 0.883,respectively.The best cut-off point for the peripheral lung lesion AT,the lesion-lung time difference were 6.5 s,2.4 sby Youden index respectively.The sensitivity and specificity were 72.9%and 85.4%,71.9%and 87.5%,respectively.Conclusions The sensitivity and specificity of the lesion-lung time difference are superior to the AT of contrast agent in the peripheral lung lesion,which has certain application value in the differential diagnosis of benign and malignant lesions.
