儿童梨状隐窝窦的超声诊断价值

摘　　要：目的探讨儿童梨状隐窝窦的高频声像图特征及临床价值。方法回顾性分析经手术及病理证实的21例梨状隐窝窦的高频超声表现,总结其声像图特点及临床价值。结果21例梨状隐窝窦中,19例来自第3鰓裂畸形,2例来自第4鰓裂畸形;18例位于左颈部,3例位于右颈部,超声上均表现为紧邻梨状隐窝旁低回声肿块,边界不清晰,形态不规则,内部回声不均匀,局部可见强回声气体,部分挤压可见内容物流动,周围组织回声增强,同侧甲状腺叶易受累移位,回声不均匀,彩色多普勒显示肿块内部无明显血流信号,周围血流信号增强。结论儿童梨状隐窝窦发生部位典型,声像图表现较特异,超声能较准确与其他颈部肿块鉴别。避免了误诊误治,为临床治疗提供可靠的影像学依据。Objective To investigate the high frequency sonographic features and clinical value of Piriform recess sinus in children.Methods The high frequency ultrasound manifestations of 21 cases of Piriform recess sinus confirmed by operation and Pathology were retrospectively analyzed,and the sonographic features and clinical value were summarized.Results In the 21 cases of Piriform recess sinus,19 were from the 3 rd branchial clefts,2 from the 4 th branchial clefts,18 in the left neck and 3 in the right neck,ultrasound showed a low echo mass adjacent to the Piriform recess sinus,the boundary was not clear,the morphology was irregular,the internal echo was uneven,strong echo gas was seen tocally and content flow can be secn by partiauy compressed mass,the surrounding tissue echo was increasel,the same side of the thyroid lobe was easy to be affected by displacement,echo uneven,there was no obvious blood flow signal inside the mass,the surrounding blood flow signal was inareased.Conclusions The location of Piriform recess sinus in children is typical,the sonographic manifestations are more specific,and the ultrasound can be more accurate in distingui shing from other neck masses.It avoids misdiagnosis and mistreatment,and pvodvies a reliable imaging basis for clinical treatment.
