摘　　要：由于目前国内尚无低血小板计数患者抗血小板方案的指南共识,临床上针对该类人群抗血小板药物使用尚缺乏经验。本文拟通过临床药师参与1例肾动脉狭窄伴低血小板计数患者的抗血小板治疗,依据患者实际情况对阿司匹林、氯吡格雷、替格瑞洛的药物选择、剂量和疗程进行个体化建议,并根据患者出血风险及用药情况选择最佳的质子泵抑制剂,协助临床医师对患者进行随访及全程药学监护,为临床上该类人群抗血小板治疗方案提供借鉴。There was no guideline nor case report of antiplatelet therapy in patients with thrombocytopenia. This paper meant to investigate antiplatelet therapy in a patient with thrombocytopenia combined renal artery stenosis. Clinical pharmacists provided individualized antiplatelet therapy for drug selection, dose adjustment and course of treatment of aspirin, clopidogel and ticagrelor depending on the patient’s specific condition, The optimal proton pump inhibitors were chosen according to the risk of bleeding and the combination use of drugs to provide whole-process pharmaceutical care during hospitalization and follow-up period. This paper provided reference for antiplatelet therapy in patients with thrombocytopenia.
