长沙市城区老年慢性病患者服药依从性影响因素研究

摘　　要：目的分析长沙市老年慢性病患者服药依从性的影响因素。方法采用问卷调查方法,对长沙市5个区县共2749例老年慢性病患者进行调查统计。计数资料采用χ^2检验分析。结果所调查的对象中,服药依从性好的占比为56.20%,依从性差的占比为43.80%。患者的文化程度、经济收入、慢病数量、服药种类、认为服药是否有利的态度等因素对患者服药依从性的影响差异有统计学意义(P <0.01);而性别、婚姻状况、同住成员、主要经济来源、锻炼情况、生病时照护者、有无医保、是否服用保健品、药品的不良反应、过敏史等因素虽然对老年患者的服药依从性有不同程度影响,但差异无统计学意义。结论提高疾病及用药知识的普及率、合理降低慢病处方药给药频次、加强临床医师及药师的社区用药跟踪工作、利用网络软件进行用药管理等有利于提高老年慢病患者服药依从性。Objective To determine the influence factors for medication compliance in elderly patients with chronic diseases in Changsha. Methods Totally 2749 elderly patients with chronic diseases in 5 administrative districts of Changsha were randomly chosen for questionnaire survey and interview. The enumeration data were managed by means of χ^2 test method. Results In the 2749 respondents, 56.20% of patients reported good compliance, and 43.80% reported poor compliance. The medication compliance was significantly influenced by the degree of education, income, numbers of chronic diseases, types of medication, and attitude to the advantage of medication(P < 0.01). But such as gender, marital status, family members, main income source, exercise situation, caregiver during sickness, with insurance or not, history of taking health food, adverse response to drugs and allergy were found to possess different effects on medication compliance, but no significant difference. Conclusion Improving the popularization rate of diseases and medication knowledge, reducing the frequency of prescription drugs for chronic diseases, strengthening the follow-up visits between patients and their doctors/pharmacists, and using network software in medication management all help improve the medication compliance in elderly patients with chronic diseases.
