经皮椎间孔镜技术结合双腔引流管灌洗引流治疗腰椎椎间隙感染临床疗效观察

摘　　要：目的:通过研究经皮椎间孔镜结合双腔引流管灌洗引流治疗腰椎椎间隙感染疾病,为腰椎感染的治疗提供新方法。方法:选取我院2016年3月-2017年12月收治的14例腰椎感染病例,予以行经皮椎间孔镜结合双腔弓丨流管治疗,术后予以敏感抗生素静脉治疗,并每天1次经留置双腔引流管行敏感抗生素灌洗,并对患者手术前后进行VAS评分(visual analogue scale,VAS)及Oswestry功能障碍指数(oswestry disability index,ODI)评价,并对结果进行分析。结果:患者于手术前,手术后拔管时及末次随访予以行上述指标进行评价,予以配对t检验进行统计学分析,术后(拔管)与术前VAS评分明显降低(P<0.05);末次随访与术前VAS评分明显降低差异(P<0.05);术后(拔管)0DI评分明显低于术前(P<0.05);末次随访0DI评分明显低于术前(P<0.05)。结论:经皮椎间孔镜技术结合双腔引流管抗生素灌洗可有效治疗腰椎椎间隙感染。Objective:To study the treatment of intervertebral space infection by percutaneous transforaminal endoscopic discectomy combined with double cavity drainage tube.Methods:A total of 14 cases of lumbar infection treated in our hospital from March 2016 to December 2017 were selected,which percutaneous transforaminal endoscopic discectomy combined with double cavity drainage tube was performed.Intravenous treatment of sensitive antibiotics after operation,indwelling double cavity drainage tube for sensitive antibiotic lavage,1 times a day.The visual analogue scale(VAS)score and Oswestry disability index(ODI)were evaluated before and after the operation,and the results were analyzed.Results:The above indicators were evaluated before and after the operation.Statistical analysis was carried out by paired t test.The difference of P value between preoperative and postoperative(extubation)VAS score was less than 0.05;the difference of P value between preoperative and last follow-up VAS was less than 0.05;the difference P value between preoperative and postoperative ODI(%)was less than 0.05;the difference P value of ODI(%)before and at the last follow-up was less than 0.05.Conclusion:Percutaneous transforaminal endoscopic discectomy technique combined with double cavity drainage tube for antibiotic irrigation can effectively treat lumbar intervertebral space infection.
