摘  要:目的探讨人外周血微小RNA 219β5p (miR-219β5p)的含量及其在糖尿病引起的视网膜内皮损伤筛查和增殖性糖尿病视网膜病变的临床诊断中的重要作用.方法共选取血浆样本156份,其中健康人血浆样本50份,无视网膜病变(non-retinopathy, NDR)的糖尿病患者血浆样本37份,非增殖型糖尿病视网膜病变( non-proliferative diabetic retinopathy, NPDR)患者血浆样本36 份,增殖性糖尿病视网膜病变(PDR)患者血浆样本33份.采用实时定量聚合酶链式反应检测miR-219β5p的表达.结果 NPDR组与PDR组相比,血浆中miR-219β5p的含量随患者视网膜损伤程度的加重而降低,两组间比较有显著统计学差异(P<0. 01).健康对照组与NDR组相比,血浆中miR-219β5p的含量无统计学差异(P>0. 05).受试者操作特征曲线(ROC)分析表明,血浆miR-219β5p对PDR有较为明显的诊断价值.健康人血浆样本的ROC曲线下面积为0. 976,敏感性为81. 21 %,特异性为90. 34 %.与健康对照组相比, NDR患者与NPDR患者的ROC曲线下面积为0. 919,敏感性为84. 75 %,特异性为94. 41 %,都与健康对照组有较为明显的差异.当诊断阈值大于或等于8. 43 时,罹患NPDR的可能性增加.当miR-219β5p含量小于或等于5. 02时,罹患PDR的可能性增大.结论 miR-219β5p检测可作为辅助诊断PDR的重要生物标志物.Objective To determine the content of microRNA 219β5p (miR-219β5p) in pe-ripheral blood and explore its application in the screening of retinal endothelial damage induced by diabetes and the diagnosis of proliferative diabetic retinopathy. Methods A total of 184 serum sam-ples were collected, with 59 from healthy subjects, 44 from from diabetic patients without diabetic retinopathy and 42 from the patients with non-proliferative diabetic retinopathy ( NPDR), 39 from patients with proliferative diabetic retinopathy ( PDR). The expression of miR-219β5p was detec-ted by real time quantitative polymerase chain reaction. Results The serum miR-219β5p was found to drop with the severity of retinal damage. There was a significant difference in miR-219β5p level between the two retinopathy groups ( P <0. 05 ). There was no significant difference in miR-219β5p level between normal people and NDR patients ( P>0. 05). The receiver operating char-acteristic curve (ROC) analysis showed that serum miR-219β5p was of significant diagnostic value for PDR. The area under the ROC curve of healthy human serum samples was 0. 976, with a sensi-tivity of 81. 21 % and a specificity of 90. 34 %. Compared with the healthy control group, the area under the ROC curve of NDR patients and NPDR patients was 0. 919, with the sensitivity being 84. 75%, and the specificity 94. 41 %, which were significantly different from those of the healthy control group. When the diagnostic threshold is greater than or equal to 8. 43, the likelihood of de-veloping NPDR is increased. When the miR-219β5p content is less than or equal to 5. 02, the pos-sibility of suffering from PDR is increased. Conclusion microRNA-219 beta 5p can serve as an im-portant biomarker helpful for the diagnosis of PDR.
