乳腺癌骨转移患者中医证候分布及组合规律研究

摘　　要：目的观察乳腺癌骨转移患者中医证候的分布及组合特征,探索其中医证型规律,为乳腺癌骨转移的预防、诊断和治疗提供依据。方法选择乳腺癌骨转移患者236例,采用《乳腺癌骨转移患者中医症状信息采集表》采集中医症状,采用SPSS21.0统计软件频数分析、因子分析、H-K聚类分析、秩和检验对数据进行分析。结果中医症状频率由高到低排序前10位为腰膝酸软、细脉、舌淡红、舌苔薄、刺痛、健忘、酸痛、涩脉、乏力、畏寒肢冷。中医证型分布情况:肝肾阴虚证组合为38.13%,脾肾阳虚证组合为35.17%,气血亏虚证组合为14.41%,气滞血瘀证组合为12.29%。结论乳腺癌骨转移的基本证型为肾虚证,不同证型均存在中医证候组合的现象。Objective To observe the features of distribution and combination of TCM syndromes in breast cancer patients with bone metastasis;To explore the laws of TCM syndromes;To provide a basis for the prevention,diagnosis and treatment of breast cancer with bone metastasis.Methods Totally 236 cases of breast cancer with bone metastasis were chosen.“TCM Symptoms Information Collection Form for Breast Cancer with Bone Metastasis”was applied to collect TCM symptoms.Statistical analysis was performed using SPSS21.0 statistical software,including frequency analysis,factor analysis,H-K cluster analysis,and rank sum test.Results The top 10(frequency from high to low)TCM symptoms were:soft waist and knee,weak pulse,pale red tongue,thin tongue coating,tingling,forgetfulness,soreness,astringent pulse,weakness,chills and cold limbs.The distribution of TCM syndromes:the combination of liver-kidney yin deficiency syndrome accounted for 38.13%,followed by the combination of spleen-kidney yang deficiency syndrome accounting for 35.17%,the combination of qi-blood deficiency syndrome accounting for 14.41%,the combination of qi-stagnation and blood-stasis syndrome accounting for 12.29%.Conclusion The basic syndrome of breast cancer with bone metastasis is kidney deficiency syndrome.The phenomenon of TCM syndrome combination exists in different TCM syndromes.
