免结扎胆囊动脉主干技术在急性结石性胆囊炎患者中的应用体会

摘　　要：目的探讨免结扎胆囊动脉主干技术用于腹腔镜胆囊切除术的安全性及临床疗效。方法回顾性分析徐州市中心医院2016年7月-2017年12月收治的84例急性结石性胆囊炎患者的临床资料。其中,观察组(n=42)实施免结扎胆囊动脉主干的腹腔镜胆囊切除术,对照组(n=42)患者行常规腹腔镜胆囊切除术。结果2组患者均顺利完成手术,无中转开腹。观察组术中出血量较对照组明显减少(P<0.05),术后并发症总发生率明显低于对照组(P<0.05).结论免结扎胆囊动脉主干的腹腔镜胆囊切除术治疗急性结石性胆囊炎安全有效,值得临床推广使用。Objective To investigate the safety and clinical efficacy of main gallbladder artery ligation - free in laparoscopic cholecystectomy. Methods The clinical data of 84 patients with acute calculous cholecystitis admitted to Xuzhou Central Hospital from July 2016 to December 2017 were retrospectively analyzed. The observation group ( n = 42 ) underwent laparoscopic cholecystectomy without ligation of the main gallbladder artery, while the control group ( n = 42) underwent routine laparoscopic cholecystectomy. Results The operation was successfully completed in both groups without conversion to laparotomy. The amount of bleeding in the observation group was significantly less than that in the control group (P <0.05), and the total incidence of complications in the observation group was significantly lower than that in the control group (P <0.05). Conclusions Laparoscopic cholecystectomy without ligation of the main gallbladder artery is safe and effective in the treatment of acute calculous cholecystitis, which is worthy of clinical application.
