同步间歇指令联合无创通气治疗COPD合并呼吸衰竭的疗效分析

摘　　要：目的探讨同步间歇指令联合无创通气治疗COPD合并呼吸衰竭的疗效。方法选取2017年6月至2018年12月在本院接受治疗的COPD合并呼吸衰竭患者50例作为研究对象,按照随机数字表法分为观察组与对照组,各25例。对照组给予适应性支持通气治疗,观察组接受同步间歇指令联合无创压力支持通气。比较两组患者治疗结束后的临床症状及体征消失时间、相关肺功能指标,及血清各项指标的浓度水平。结果观察组患者临床症状及体征消失时间均低于对照组(P<0.05);观察组患者治疗后VC、FVC、MVV肺功能指标均高于对照组(P<0.05);观察组患者治疗后血清CRP、IL-6、TNF-α均优于对照组(P<0.05)。结论采用同步间歇指令联合无创通气治疗COPD合并呼吸衰竭患者有更加显著的临床疗效,改善各项功能指标,促进患者早日康复。Objective To explore the effect of synchronous intermittent instruction combined with noninvasive ventilation on COPD patients with respiratory failure.Methods Fifty of COPD patients with respiratory failure treated in our hospital from June 2017 to December 2018 were selected as the study subjects.They were divided into observation group and control group according to the random number table method,with 25 cases in each group.The control group was given adaptive support ventilation treatment.The observation group received synchronous intermittent instruction combined with non-invasive pressure support ventilation.The disappearance time of clinical symptoms and signs,related pulmonary function indexes and serum indicators were compared between the two groups.Results The disappearance time of clinical symptoms and signs in the observation group was lower than that in the control group (P<0.05).The pulmonary function indexes of VC,FVC and MVV in the observation group were higher than those in the control group after treatment (P<0.05).After treatment,the serum CRP,IL-6 and TNF-α in the observation group were better than those in the control group (P<0.05).Conclusion Synchronized intermittent instruction combined with non-invasive ventilation for COPD patients with respiratory failure has more significant clinical efficacy,improve the functional indicators,and promote the early recovery of patients.
