摘  要:目的 探讨婴幼儿先天性心脏病围术期中动脉血乳酸变化的临床意义。方法 选择2018年1月-2018年12月实施体外循环手术的婴幼儿先天性心脏病176例,动态监测从麻醉后、术中到术后48h内的动脉血乳酸值。结果 176例患者动脉血乳酸值从麻醉结束到体外循环停机时呈现连续上升态势,到体外循环停机时达到最高。数据显示,任何时间点位的动脉血乳酸浓度高于5.0mmol/L,提示可能预后不良。结论 动态监测婴幼儿先天性心脏病围术期中动脉血乳酸的变化,可作为指导治疗的有效指标之一。Objective To observe the concentration of lactic acid of artery blood and to evaluate its effect during perioperative of infants with congenital heart disease(CHD) under cardiopulmonary bypass (CPB) operation. Methods One hundred and sevsnty-six CHD cases from Jan 2018 to Dec 2018 were reviewed. Dynamic monitoring of the concentration of lactic acid of artery blood after cardiac operation under CPB. Results The concentration of lactic acid of artery blood increased gradually from anaesthesia to end of CPB,and it reached the highest level at end of CPB. If the concentration of lactic acid of artery blood is higher than 5.0 mmol/L at any time during the perioperation, a bad prognosis of the patient may be indicated. Conclusion The concentration of lactic acid of artery blood in the perioperation for infant with CHD is reliable and effective prognosis indicator.
