摘　　要：目的探讨Salter骨盆截骨+股骨短缩旋转截骨治疗儿童发育性髋脱位的疗效。方法 对21例发育性髋脱位患儿采用Salter骨盆截骨+股骨短缩旋转截骨治疗,观察患儿治疗前后的髋臼指数,评价髋关节功能优良率,评价头、臼形态与相互关系及测量C-E角的优良率。结果 患儿获得1~4年随访。治疗后患者的髋臼指数为21.57±0.24,明显低于治疗前的35.02±0.95(P<0.05)。髋关节功能优良率治疗后1年为20/21(95.24%),明显高于治疗前的3/21(14.29%)(P<0.01)。头、臼形态与相互关系及测量C-E角的优良率治疗后为19/21(90.48%),明显高于治疗前的2/21(9.52%)(P<0.01)。结论 Salter骨盆截骨+股骨短缩旋转截骨治疗儿童发育性髋脱位具有显著的临床效果。Objective To investigate the efficacy of Salter pelvic osteotomy and femoral shortening rotational osteotomy in the treatment of developmental hip dislocation in children. Methods The 21 children with developmental dislocation of the hip were treated with Salter pelvic osteotomy plus femoral shortening rotational osteotomy.The pre- and post-operative acetabular index,excellent and good rate of hip joint function,and excellent and good rate of shape and mutual relationship of head and acetabulum,measuring C-E angle in the two groups of patients were observed and evaluated. Results All patients were followed up for 1~4 year.After treatment,the acetabular index of the patients was 21.57±0.24,which was significantly lower than 35.02±0.95 of the preoperative ( P <0.05).The excellent and good rate of hip function was 20/21(95.24%) at 1 year after treatment,which was significantly higher than 3/21(14.29%) before treatment ( P <0.01).The excellent and good rate of head and acetabulum morphology,mutual relationship and measurement of C-E angle was 19/21(90.48%) after treatment,which was significantly higher than 2/21 (9.52%) before treatment ( P <0.01). Conclusions Salter pelvic osteotomy and femoral shortening rotational osteotomy have significant clinical effects in the treatment of developmental hip dislocation in children.
