早期急性化脓性膝关节炎持续闭式引流灌洗与间断注射器冲洗的疗效比较

摘　　要：目的比较间断注射器冲洗和持续闭式引流灌洗配合静脉抗生素治疗早期急性化脓性膝关节炎的疗效。方法 将61例早期急性化脓性膝关节炎患者按入院顺序依次分为A组(间断注射器冲洗,31例)和B组(持续闭式引流灌洗,30例)。比较两组住院时间、住院费用、出院时好转率、治愈率和复发率。结果 患者均获得随访,时间12~48个月。住院费用A组少于B组(P<0.01);住院时间、出院时好转率、治愈率和复发率两组比较差异均无统计学意义(P>0.05)。结论 间断注射器冲洗和持续闭式引流灌洗配合静脉抗生素治疗早期急性化脓性膝关节炎,住院时间、出院时好转率、治愈率和复发率无明显差异,但住院费用前者明显减少,具有明显优势。Objective To compare the curative effect of continuous closed drainage lavage combined with intravenously guttae of antibiotics and intermittent syringe flushing in early acute septic arthritis of the knee. Methods The 61 patients with early acute septic arthritis of knee were divided into group A(intermittent syringe flushing,31 cases) and group B(continuous closed drainage lavage,30 cases) according to the sequence of admission.The length of hospital stay,hospitalization expenses,improvement rate at discharge,cure rate and recurrence rate were compared between the two groups. Results Patients were followed up for 12~48 months.The hospitalization expenses in group A was less than that in group B ( P <0.01);there were no significant differences in the length of hospital stay,improvement rate at discharge,cure rate and recurrence rate between the two groups ( P >0.05). Conclusions Intermittent syringe flushing and continuous closed drainage lavage combined with intravenously guttae of antibiotics therapy for early acute septic arthritis of the knee show no significant difference in the length of hospital stay,improvement rate at discharge,cure rate and recurrence rate,but the former significantly reduce hospitalization expenses and have obvious advantages.
