摘  要:目的探讨兔腰椎间盘退变髓核中Th17/Treg细胞相关因子IL-17及TGF-β的变化情况。方法 新西兰大白兔50只,随机取20只作为对照组;剩余30只采用腹前外侧入路穿刺L3~4、L4~5椎间盘建立腰椎退变动物模型,MRI检查确定椎间盘退变,选取造模成功的20只作为实验组。第4、8周两组分别处死10只动物,观察椎间盘的大体观。第4、8周HE染色对两组动物的椎间盘髓核进行病理观察,从炎性细胞浸润、纤维组织增生及新生血管形成3个方面加以评估。第4、8周采用ELISA检测两组椎间盘中IL-17及TGF-β含量。结果 实验组与对照组第4、8周炎性细胞浸润、新生血管形成、纤维组织增生比较差异有统计学意义(P<0.01),实验组第4、8周炎性细胞浸润、纤维组织增生及新生血管形成比较差异有统计学意义(P<0.01)。第4、8周实验组中IL-17、TGF-β含量比对照组明显增多(P<0.01)。结论 腰椎间盘突出可能与自身免疫反应有关。Objective To investigate the changes of serum level of IL-17 and TGF-β in the nucleus pulposus of the lumbar disc degeneration. Methods Fifty New Zealand rabbits were involved in this study, including 20 rabbits were randomly selected as the control group. The remaining 30 ones underwent anterior ventrolateral approach for L 3~4 and L 4~5 intervertebral discs to establish animal models of lumbar degeneration,which were confirmed the degeneration of intervertebral discs with MRI examination, and then 20 successful models were selected as the experimental group. Ten rabbits were sacrificed to observe the intervertebral disc at 4,8 weeks respectively. HE staining was performed to observe the pathological changes of intervertebral disc nucleus in each group, which included three aspects: inflammatory cell infiltration, fibrous tissue hyperplasia and neovascularization. Meanwhile the contents of IL-17 and TGF-β of intervertebral disc tissues in two groups were detected with ELISA. Results At 4,8 weeks,there were significant differences in inflammatory cell infiltration,neovascularization and fibroplasia between the experimental group and the control group ( P <0.01);inflammatory cell infiltration, fibrous tissue proliferation and neovascularization were also increased obviously in the experimental group ( P <0.01);the contents of IL-17 and TGF-β in degenerated nucleus pulposus were significantly higher in the experimental group than those in control group ( P <0.01). Conclusions Lumbar disc herniation may be related to autoimmune response.
