摘 要:目的通过解剖同一个体的左右侧前交叉韧带(ACL)胫骨骨性止点,对比双侧形态及大小是否存在差异,以验证能否使用对侧ACL胫骨骨性止点资料进行计算机辅助系统下的ACL胫骨隧道个性化重建。方法 收集10例行双侧全膝关节置换术患者的胫骨平台标本,对ACL胫骨残端纤维进行解剖,观察左右侧胫骨骨性止点的形态,测量该骨性止点的长度、宽度、深度、面积,对比左右侧ACL胫骨骨性止点的差异。结果 左右侧ACL胫骨止点的长度、宽度、深度、面积比较差异无统计学意义(P≥0.05),且长度(r=0.820,P=0.004)和面积(r=0.802,P=0.005)呈显著相关性;但在外观形态方面差异较大。结论 支持使用对侧ACL胫骨骨性止点的形态学测量结果作为计算机辅助系统下ACL胫骨隧道重建的参考资料,但在移植物形态选择方面,不建议将对侧ACL胫骨骨性止点的形态外观作为参考资料。Objective To compare the side differences in size and shape of bony tibial insertion of anterior cruciate ligament(ACL) by dissecting the bilateral tibial plateaus from the same subject,then validate whethert it can use the contralateral bony tibial insertion of ACL datas to perform computer-assisted ACL tibial tunnel individual reconstruction or not. Methods A total of 20 resected tibial plateaus retrieved from 10 patients who underwent bilateral total knee arthroplasty surgery;dissected the ACL tibial stumps,observed the shape of the bony tibial insertion,measured its length,width,depth and areas.Finally,compared side differences between the left and right bony tibial insertions of ACL. Results There were no significant differences in length,width, depth and area between the right and left bony tibial insertions of ACL in the same subject( P ≥0.05),moreover,the length( r =0.820, P =0.004) and area ( r =0.802, P =0.005)showed significant correlation.However,the shape revealed a large difference. Conclusions This study supports to obtain morphometric parameters from contralateral bony tibial insertion of anterior cruciate ligament as reference datas to perform computer-assisted tibial ACL tunnel individual reconstruction,however,as for the selection of graft′s shape,it does not recommend.
