摘  要:目的探究福辛普利与厄贝沙坦联合对大鼠实验性心肌缺血再灌注损伤(MIRI)模型血流动力学、血清心肌酶学和SOD、CAT、GSH-Px活性的影响。方法 Wister大鼠随机分为假手术组、模型组、福辛普利组(20 mg/kg)、厄贝沙坦组(100 mg/kg)、联合用药组(10 mg/kg+50 mg/kg)。模型组及给药组大鼠建立MIRI模型,记录各组大鼠左心室收缩压(LVSP),左心室内压最大上升和下降速率(±dp/dtmax),动脉收缩压(SBP)、动脉舒张压(DBP)、心率(HR)、并计算平均动脉压(MAP);检测血清谷草转氨酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)及红细胞膜ATPase活性;计算心肌梗死面积(MIS);测定血清超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、谷胱甘肽过氧化物酶(GSH-Px)活性。结果与模型组比较,福辛普利组、厄贝沙坦组及联合用药组大鼠MIS,SBP、DBP、MAP水平及AST、LDH、CK-MB活性显著降低,LVSP,±dp/dtmax及SOD、CAT、GSH-Px、Ca2+-ATP酶活性显著升高(P<0.05),而HR无明显差异(P>0.05)。结论低剂量福辛普利与厄贝沙坦联合治疗能有效改善MIRI大鼠血流动力学、降低血清心肌酶释放,增强抗氧化酶活性,与单纯应用高浓度福辛普利或厄贝沙坦治疗的疗效相当。Objective To explore the effects of fosinopril combined with irbesartan on hemodynamics, serum myocardial enzymology and SOD, CAT and GSH-Px activities in rats with experimental myocardial ischemia-reperfusion injury(MIRI). Methods Wister rats were randomly divided into sham operation group, model group, fosinopril group(20 mg/kg), irbesartan group(100 mg/kg) and combined group(10 mg/kg+50 mg/kg). The MIRI models were established in model group and drug administration group. The left ventricular systolic pressure(LVSP), the maximum increase and decrease rates of left ventricular pressure(±dp/dtmax), arterial systolic blood pressure(SBP), arterial diastolic blood pressure(DBP) and heart rate(HR) were recorded among the groups, and the mean arterial pressure(MAP) were calculated. The serum aspartate aminotransferase(AST), lactate dehydrogenase(LDH), creatine kinase isoenzyme(CK-MB) and erythrocyte membrane ATPase activities were detected. The myocardial infarct size(MIS) was calculated. The serum superoxide dismutase(SOD), catalase(CAT) and glutathione peroxidase(GSH-Px) activities were determined. Results Compared with model group, the levels of MIS, SBP, DBP and MAP and the activities of AST, LDH and CK-MB were significantly decreased in fosinopril group, irbesartan group and combined group, and the LVSP,±dp/dtmax, SOD, CAT, GSH-Px and Ca2+-ATPase activities were significantly increased(P<0.05), but there was no significant difference in HR(P>0.05). Conclusion Low dose fosinopril combined with irbesartan can effectively improve hemodynamics, decrease myocardial enzyme release and enhance antioxidant enzyme activity in MIRI rats. And it has comparable efficacy with simple high-concentration of fosinopril or irbesartan.
