摘 要:目的比较反流性咽喉炎(laryngo-pharyngealref lux,LPR)患者人群与正常人群的喉部上皮细胞形态和黏膜防御、炎症相关基因表达差异。方法采集LPR组(80例)和对照组(36例)喉部的4个不同部位(真声带、假声带、内侧杓状软骨、后连合)组织,镜下观察细胞形态,并采用聚合酶链式反应方法对19个喉部上皮细胞形态、黏膜防御和炎症相关基因表达进行分析,比较喉部4个不同部位的基因表达水平。结果内侧杓状软骨区LPR组与对照组相比KRT14表达差异有统计学意义,与对照组相比炎症相关基因CRNN、CD1d和TGFβ-1差异有统计学意义;LPR组和对照组黏膜防御相关基因在内侧杓状软骨区(MUC2、MUC5B和CDH-1)和后连合区(MUC5B)表达差异有统计学意义。结论 LPR组与对照组存在上皮细胞形态和基因表达的改变,基因表达的变化将可能作为LPR的诊断指标。OBJECTIVE To compa r e the difference of laryngeal epithelial cells morphology and mucosal defense and inflammation-related gene expression between laryngopharyngeal ref lux(LPR) patients and normal subjects. METHODS Those who met the inclusion criteria were divided into LPR group(n=80) and control group(n=36). The specimen tissues from four sub-sites of the larynx were collected in LPR group and control group respectively. The specimen tissues from four sub-sites(true vocal cords, false vocal cords, medial arytenoid cartilage, posterior commissure) were examined by microscopic examination, and 19 mucosal defense and inflammationrelated gene expression were performed by polymerase chain reaction(qRT-PCR). The gene expression levels of the four sub-sites of the larynx were compared. RESULTS The expression of KRT14 was significantly different between the LPR group and the control group in the medial arytenoid cartilage region. The expression of CRNN, CD1d and TGFβ- 1 in the LPR group was significantly different from that in the control group in the medial arytenoid cartilage region. There was a significant difference among the expression of MUC2, MUC5B, and CDH-1in medial arytenoid cartilage region and MUC5B in posterior commissure between the LPR group and control group. CONCLUSION There are changes in morphology and gene expression of epithelial cells in LPR group and control group, and the change of gene expression may be used as a diagnostic index of LPR.
