摘  要:目的探讨白细胞介素(IL)-ll与脓毒症心肌损伤预后的关系.方法选择2017年1月至2018年6月我院重症医学科收治的44例脓毒症心肌损伤的患者,根据患者的临床结局分为生存组(28例)和死亡组(16例).收集各组患者的临床资料,并检测血清IL-11水平.通过脉搏指示连续心排血量监测技术(PiCCO)监测患者的血流动力学.结果与生存组相比,死亡组患者心脏指数[(3.2±0.4)L·min^-1·m^-2比(2.6±0.3)L.min^-1·m^-2]、心功能指数[(4.6±0.8)L/min比(3.9±0.5)L/min]、全心射血分数[(16.5±1.6)%比(14.7±1.3)%]和左心室收缩力指数[(1349±285)比(1024±231)]等指标显著降低,差异有统计学意义(P<0.05);与生存组相比,死亡组患者SOFA评分[(5.3±1.6)分比(8.2±2.3)分]和IL-11水平[(25.6±8.3)ng/ml比(46.1±10.7)ng/ml]显著升高,差异有统计学意义;ROC曲线分析结果表明,血清IL-11对脓毒症心肌损伤患者预后具有一定的预测价值(P<0.05),其AUC为0.883,诊断敏感度和特异度分别为73.7%和80.5%.结论血清IL-11水平有助于预测脓毒症心肌损伤患者的预后.Objective To investigate the relationship between interleukin (IL)-11 and the prognosis of sepsis-induced myocardial injury. Methods A total of 44 sepsis-induced myocardial injury patients admitted in the department of critical care medicine of Renmin Hospital of Wuhan University. The patients were divided into the survival group (n=28) and the death group (n= 16) according to the clinic outcome. The clinical data of each group were collected and the levels of IL-11 were measured. The hemodynamic monitoring was performed with pulseindicated continuous cardiac output(PiCCO). Results Compared with survival group, the level of cardiac index [(3.2±0.4) L . min^-1 . m^-2 vs.(2.6±0.3) L . min^-1 . m^-2], cardiac function index [(4.6±0.8)L/min vs.(3.9±0.5) L/min], global ejection fraction [(16.5± 1.6)% vs.(14.7±1.3)%], dP/dPmax [(1349±285) vs.(1024±231)] were significantly decreased in death group (P<0.05). Moreover, the sequential organ failure assessment (SOFA) score [(5.3 ± 1.6) vs.(8.2 ±2.3)] and the level of IL-11 [(25.6 ±8.3) ng/ml vs.(46.1 ± 10.7) ng / ml ] were significantly up-regulated in the death group (P<0.05). Receiver operating characteristic curve analysis revealed that an area under the curve was 0.883丄 susceptibility and specificity of IL-11 were 73.7% and 80.5% respectively. Conclusion The IL-11 may be helpful to predict the prognosis of sepsis-induced myocardial injury.
