动态心电图、平板运动试验及心脏远程监护技术诊断心肌缺血的价值对比

摘　　要：目的比较动态心电图、平板运动试验及心脏远程监护技术在心肌缺血诊断中的应用效果。方法选取2018年2月至11月在长治市中医院进行冠心病治疗的94例患者,分别对其采用动态心电图、平板运动试验及心脏远程监护3种检查方法进行诊断,并将其检查结果与冠状动脉造影检查结果进行对比,比较3种检查的敏感性、特异性及准确率。结果经冠状动脉造影检查显示,心肌缺血阳性者67例,阴性者27例;动态心电图检查的敏感性、特异性及准确率分别为83.58%、51.85%及74.47%,平板运动试验检查的敏感性、特异性及准确率分别为88.06%、59.26%及79.79%,心脏远程监护检查的敏感性、特异性及准确率分别为94.03%、81.48%及90.43%,心脏远程监护检查的效果优于动态心电图及平板运动试验(P<0.05)。心脏远程监护检查的阳性预测值及阴性预测值分别为92.65%、84.62%,其检查效果优于动态心电图及平板运动试验(P<0.05)。结论与动态心电图、平板运动试验相比,采用心脏远程监护技术对心肌缺血患者进行诊断,其诊断准确率更优秀,且该检查方法便捷度更高,值得临床推广使用。Objective To compare the application effects of dynamic electrocardiogram, treadmill exercise test and cardiac remote monitoring technique in diagnosis of myocardial ischemia. Methods A total of 94 patients who received treatment for coronary heart disease in Changzhi Hospital of Traditional Chinese Medicine from February 2018 to November 2018 were enrolled. They were diagnosed by dynamic electrocardiogram, treadmill exercise test and cardiac remote monitoring. The results were compared with those of coronary angiography, and the sensitivity, specificity and accuracy of the three tests were compared. Results Coronary angiography showed that 67 cases were positive for myocardial ischemia and 27 cases were negative for myocardial ischemia. The sensitivity, specificity and accuracy of dynamic electrocardiogram were 83.58%, 51.85% and 74.47%, respectively;the sensitivity, specificity and accuracy of treadmill exercise test were 88.06%, 59.26% and 79.79% respectively;the sensitivity, specificity and accuracy of cardiac remote monitoring were 94.03%, 81.48% and 90.43%, respectively. The effects of cardiac remote monitoring were better than those of dynamic electrocardiogram and treadmill exercise test (P<0.05). The positive predictive value and negative predictive value of cardiac remote monitoring were 92.65% and 84.62% respectively, which were better than those of dynamic electrocardiogram and treadmill exercise test (P<0.05). Conclusions Compared with dynamic electrocardiogram and treadmill exercise test, the accuracy of cardiac remote monitoring technology for the diagnosis of myocardial ischemia is better, and the method is more convenient, and worth promoyion in clinical.
