长程动态心电图在心律失常诊断中应用

摘　　要：目的探讨长程单导联动态心电图在心律失常诊断中的应用。方法选取自2017年12月至2018年12月北部战区总医院心血管内科门诊收治的32例患者为研究对象,临床表现为心悸、头晕、黑蒙或晕厥,行24 h动态心电图检查后未发现与主要不适症状相关的心律失常或诊疗证据尚不充分者,再行长程单导联动态心电图检查7~14 d,分析并比较24 h动态心电图与长程动态心电图在心律失常事件诊断中的检出率,同时观察患者对长程动态心电图佩戴的舒适度及依从性。结果长程动态心电图检查有效时长百分比为98.31%(95.55%~99.31%),24 h动态心电图检查有效时长百分比为99.37%(98.75%~100.00%),组间比较,差异有统计学意义(P<0.05)。长程动态心电图检出心律失常阳性率为68.8%(22/32),高于24 h动态心电图检查的15.6%(5/32),差异有统计学意义(P<0.05)。长程动态心电图记录的图像清晰,佩戴更舒适,患者依从性更好。结论长程单导联动态心电图对心律失常诊断率高,患者依从性好。Objective To investigate the application of long-term single-lead dynamic electrocardiogram(DCG)in the diagnosis of arrhythmia.Methods A retrospective study was performed on 32 cases of patients who were admitted from December 2017 to December 2018 in the General Hospital of Northern Theater Command.Clinical manifestations of heart palpitations,dizziness,amaurosis or syncope,row after 24 hours dynamic electrocardiogram inspection found no arrhythmias associated with major symptoms or clinical evidence is insufficient,again long range of single lead dynamic ecg examination 7-14 days,analysis and compare the 24 hours DCG and long range rate of DCG in the diagnosis of arrhythmia events,at the same time observe patients for long-term DCG wearing comfort and compliance.Results The percentage of effective duration of long-distance DCG examination was 98.31%(95.55%-99.31%),and the percentage of effective duration of 24 hours DCG examination was 99.37%(98.75%-100.00%),and the difference between groups was statistically significant(P<0.05).The positive rate of arrhythmia detected by long range DCG was 68.8%(22/32),higher than that of 15.6%(5/32)by 24 hours DCG(P<0.05).The image recorded by long range dynamic electrocardiogram was clear,more comfortable to wear,and patient compliance was better.Conclusion The rate of diagnosis of arrhythmia is high and the patient compliance is good.
