临床药师参与一例血液透析患者抗感染治疗的实践与体会

摘　　要：目的探讨临床药师参与血液透析合并感染患者实施药学监护的意义。方法临床药师对1例血液透析合并感染患者的抗感染药物选择、用法用量、给药时机给予个体化的用药指导及全程的药学监护。结果经过抗感染治疗,患者感染灶得到了有效控制。结论临床药师可发挥药学专业优势,协助医师提高药物治疗的安全性和有效性。Objective To explore the significance of clinical pharmacist participating in the implementation of pharmaceutical care for a case of patient with hemodialysis and infection.Methods The clinical pharmacist gave one case of hemodialysis patient individualized medication guidance and full-scale pharmaceutical monitoring on the anti-infective drug selection,usage and dosage,and timing of administration.Results After anti-infective treatment,the patient's infection was effectively controlled.Conclusion Clinical pharmacists can take advantage of pharmacy to help physicians improve the safety and effectiveness of drug treatment.
