摘　　要：目的探讨血管内斑块减容术联合药物涂层球囊(DCB)治疗下肢动脉硬化闭塞症(ASO)的临床效果。方法选择采用血管内斑块减容术(TurboHawk系统)联合DCB治疗的20例下肢ASO患者为研究对象,术后1、3、6个月复查其踝肱指数(ABI)、下肢彩超或CT血管成像,评估治疗效果。结果 20例患者下肢动脉病变段均开通成功,其中1例出现远端栓塞者经股动脉切开Forgaty导管取栓成功,1例出现动脉穿孔,予以Viabahn覆膜支架补救成功,技术成功率为90%;患者术后缺血症状明显改善,术后1、3、6个月的ABI均高于术前(P<0.05)。结论血管内斑块减容术联合DCB是治疗下肢ASO的一种新方法,其具有创伤小、安全有效、无植入物等优势,值得临床推广应用。Objective To investigate the clinical effect of intravascular plaque volume reduction surgery combined with drug-coated balloon (DCB) in the treatment of lower extremity arteriosclerosis obliterans (ASO). Methods Twenty patients with lower extremity ASO treated by intravascular plaque volume reduction surgery (TurboHawk system) combined with DCB were selected as the study objects. The ankle brachial index (ABI), color Doppler ultrasound or CT angiography of lower extremity were reexamined at 1, 3 and 6 months after operation to evaluate the therapeutic effect. Results The lesion segments of lower extremity arteries were successfully opened in 20 patients. One case of distal embolism was successfully removed in femoral artery embolectomy by Forgaty catheter, and one case of artery perforation was successfully repaired by Viabahn covered stent, the technical success rate was 90%. The ischemia symptoms of patients improved significantly after operation, the ABI of 1, 3 and 6 months after operation were higher than that before operation (P<0.05). Conclusion Intravascular plaque volume reduction surgery combined with DCB is a new method for the treatment of lower extremity ASO. It has the advantages of small trauma, safety and effectiveness, and no implant, which is worthy of clinical promotion and application.
