摘　　要：目的研究环乳晕切口对男性乳腺增生症患者乳晕区感觉神经的影响。方法纳入我院收治的男性乳腺增生患者84例,按照手术方法的不同将其分为A组(40例)和B组(44例)。A组患者采用环乳晕下缘弧形切口治疗,B组患者采用环乳晕环内侧弧形切口治疗。比较两组患者术后乳晕区感觉情况和围术期资料。结果A组痛觉减退、触觉减退和皮肤异样感的发生率均明显高于B组(P<0.05)。两组均未出现乳头坏死和复发情况,A组术中出血量明显少于B组(P<0.05)。两组术后引流时间、血肿、疼痛发生率和舒适度评分比较,差异无统计学意义(P>0.05)。结论采用环乳晕内侧弧形切口治疗男性乳腺增生症效果满意,能明显降低对乳晕处神经的损伤且安全性高,患者术后舒适度良好,值得在临床中应用。Objective To study the effect of circum-areolar incisions on sensory nerve in areola area of male patients with mammary hyperplasia. Methods A total of 84 male patients with mammary hyperplasia admitted in our hospital were enrolled. The patients were divided into group A (40 cases) and group B (44 cases) according to different surgical methods. The group A was treated with arc incision at the lower edge of circum-areola, while the group B was treated with arc incision at medial circum-areola. The sensation of areola area and perioperative data were compared between the two groups. Results The incidences of hypoalgesia, hypotactility and skin abnormality in the group A were significantly higher than those of the group B (P<0.05). No nipple necrosis and recurrence occurred in both groups. The intraoperative bleeding volume in the group A was significantly less than that in the group B (P<0.05). There were no significant differences in drainage time, incidences of hematoma and pain and comfort score between the two groups (P>0.05). Conclusion The arc incision at medial circum-areola has satisfactory effect in the treatment of male mammo-hyperplasia, which can significantly reduce the areola nerve damage and has higher safety, with good postoperative comfort. It is worthy of clinical application.
