滋阴降火法治疗围绝经期中重度潮热患者潮热及肱动脉FMD临床观察

摘　　要：目的观察滋阴降火法对肾阴虚型围绝经期中重度潮热的临床疗效及其对肱动脉FMD的影响,由此探讨滋阴降火法缓解围绝经期中重度潮热的作用机制及其对女性绝经后CVD(心血管疾病)的预防作用,体现中医“上工治未病”的思想。方法选择肾阴虚型绝经期中重度潮热患者31例,给予滋阴降火法中药连续治疗3个月,以国内改良的Kupperman评分、肱动脉FMD作为观察指标,进行治疗前后的自身对照。结果用药前后围绝经期潮热临床症状积分差异有统计学意义(P<0.05)。治疗后,31例患者的FMD%水平较疗前整体有所升高(P<0.01)。结论滋阴降火法治疗肾阴虚型绝经期中重度潮热疗效显著,能有效改善血管内皮功能,从而为滋阴降火法缓解围绝经期中重度潮热的作用机制,探讨滋阴降火法预防绝经后CVD的有效性提供依据。Objective To observe the clinical effect of nourishing yin and reducing fire on moderate and severe hot flashes in peri-menopausal syndrome of kidney yin deficiency type and its effect on brachial artery FMD (brachial artery blood flow-mediated dilation),and to explore the therapeutic effect and mechanism of nourishing yin and reducing fire on moderate and severe hot flashes in patients with kidney yin deficiency type perimenopausal syndrome and its preventive effect on cardiovascular disease (CVD). Methods 31 cases of moderate and severe hot flashes in menopausal syndrome of kidney yin deficiency type were slected,and the treatment of nourishing yin and reducing fire for 3 months was carried out. The improved Kupperman scores and brachial artery FMD were used as the index of observation self-control before and after treatment. Results There was significant difference in the clinical symptom score of peri-menopausal hot flashes before and after treatment (P<0.05). After treatment,the FMD% level in 31 patients was higher than that before treatment (P < 0.01). Conclusion Nourishing yin and reducing fire is effective in treating moderate and severe hot flashes in peri-menopausal syndrome of kidney yin deficiency type,effectively improve vascular endothelial function,so as to alleviate clinical symptoms and prevent postmenopausal CVD.
