哌拉西林钠加胰岛素局部换药治疗糖尿病足的疗效观察

摘　　要：目的:观察哌拉西林钠加胰岛素局部换药治疗糖尿病足的疗效。方法:选取2015年5月至2018年5月年我院收治的52例糖尿病足患者,采用抽签法将其随机分成两组各26例,对照组给予患者胰岛素局部换药治疗,经0.5%碘伏创面消毒后,将胰岛素8IU均匀滴于局部溃疡处,观察组采用哌拉西林钠加胰岛素局部换药治疗,将哌拉西林钠均匀涂抹于局部溃疡处,再用胰岛素8IU均匀滴在患处,两组均使用无菌纱布包扎,治疗30d后比较临床疗效,统计患者创面愈合时间及不良反应发生情况。结果:观察组患者创口愈合时间(13.27±3.26)d明显短于对照组的(27.49±2.82)d(P<0.05);观察组治疗有效率(92.31%)明显高于对照组(69.23%)(P<0.05);观察组并发症发生率(11.54%),对照组并发症发生率(7.69%),组间不良反应发生率数据对比无明显差异(P>0.05)。结论:哌拉西林钠加胰岛素局部换药治疗糖尿病足可有效改善患者临床症状,促进创口愈合,降低病足溃疡病变等级,无严重不良反应发生,安全性良好,治疗效果显著。Objective:To observe the efficacy of piperacillin sodium plus insulin in the treatment of diabetic foot.Methods:52 patients with diabetic foot were randomly divided into two groups by lottery from May 2015 to May 2018.The control group was treated with insulin local dressing change.After 0.5%iodophor wound disinfection,insulin 8U was evenly dripped into local ulcer.The observation group was treated with piperacillin sodium and islets of pancreas.Piperacillin sodium was evenly smeared on the local ulcer and insulin 8 u was dripped on the affected area.Both groups were dressed with aseptic gauze.After 30 days of treatment,the clinical efficacy was compared and the healing time and adverse reactions were counted.Results:The wound healing time of the observation group was significantly shorter than that of the control group(13.27±3.26)days(27.49±2.82)days(P<0.05);the effective rate of the observation group(92.31%)was significantly higher than that of the control group(69.23%)(P<0.05);the incidence of complications in the observation group(11.54%)and the incidence of complications in the control group(7.69%);and the incidence of adverse reactions between the two groups was not statistically significan(P>0.05).Conclusion:Piperacillin sodium plus insulin local dressing change can effectively improve the clinical symptoms of diabetic foot,promote wound healing,reduce the grade of ulcer lesions,no serious adverse reactions,good safety,significant therapeutic effect.
