原发性骨质疏松症中医辨证分型研究

摘　　要：目的:系统整理中医药辨证治疗原发性骨质疏松症文献和流行病学调查文献基础上探讨原发性骨质疏松症临床常见辨证分型,为提高中医药治疗原发性骨质疏松症的临床疗效提供理论依据。方法:调查文献以中国期刊全文数据库(CNKI)2007年1月-2016月12月为主,辅以中国中医药数据库检索系统(2007年1月-2016年12月)。以“骨质疏松”为主题词检索,筛选中医药治疗原发性骨质疏松症的文献和基于流行病学调查的原发性骨质疏松症证型研究文献。对符合纳入、排除标准的中医药治疗原发性骨质疏松症临床文献进行整理,并按照采用频数分析法进行分析。结果:累积频率25%以上的证型由小到大依次为:脾肾阳虚、肾阴阳两虚、脾胃虚弱、肾阳虚、肝肾阴虚、肾虚血瘀、血瘀气滞、肾精亏虚。结论:原发性骨质疏松症的临床常见证型分别为脾肾阳虚、肾阴阳两虚、脾胃虚弱、肾阳虚、肝肾阴虚、肾虚血瘀、血瘀气滞、肾精亏虚。原发性骨质疏松症的基本病机是肾阴亏虚。Objective: To systematically sort out the literature and epidemiological investigation of dialectical treatment of primary osteoporosis in Chinese medicine, and to explore the clinical syndrome differentiation of primary osteoporosis. To provide a theoretical basis for improving the clinical efficacy of TCM on primary osteoporosis. Methods: The CNKI was mainly used, from January 2007 to December 2016, supplemented by the Chinese Medicine Database Search System, also from January 2007 to December 2016. The key words on “osteoporosis” was used to search for the literature on the treatment of primary osteoporosis with TCM and the research literature on primary osteoporosis based on epidemiological investigation. The clinical literature on the treatment of primary osteoporosis in accordance with the inclusion and exclusion criteria was collated and analyzed according to the frequency analysis method. Results: The cumulative frequency of more than 25% of the syndromes from little to great in order: Pishen Yangxu (脾肾阳虚), Shen Yinyang Liangxu (肾阴阳两虚), Piwei Xuruo (脾胃虚弱), Shenyangxu (肾阳虚), Ganshen Yinxu (肝肾阴虚), Shenxu Xueyu (肾虚血瘀), Xueyu Qizhi (血瘀气滞) and Shenjing Kuixu (肾精亏虚). Conclusion: The common clinical syndromes of primary osteoporosis are Pishen Yangxu, Shen Yinyang Liangxu, Piwei Xuruo, Shenyangxu, Ganshen Yinxu, Shenxu Xueyu, Xueyu Qizhi and Shenjing Kuixu. The basic pathogenesis of primary osteoporosis is Shenjing Kuixu.
