输卵管开窗术、切除术治疗重度输卵管积水不孕症的效果比较

摘　　要：目的 探讨输卵管开窗术、输卵管切除术对重度输卵管积水不孕症患者卵巢储备功能及体外受精-胚胎移植(IVF-ET)成功率的影响。 方法 回顾性分析2015年12月至2018年9月期间在本院进行手术治疗的118例重度输卵管积水不孕症患者的临床资料,并根据手术方式不同分为两组:输卵管开窗术组57例、输卵管切除术组61例。比较两组患者术后的卵巢储备情况及IVF-ET妊娠结局。 结果 输卵管开窗术组患者术后基础窦状卵泡数、HCG注射日成熟卵泡数显著高于输卵管切除术组患者(P<0.05);两组患者的卵巢体积、移植日子宫内膜厚度比较无显著性差异(P>0.05)。两组患者术后血清E2、FSH、LH水平比较无显著性差异(P>0.05)。两组患者的胚胎着床率、临床妊娠率、早期流产率比较亦无显著性差异(P>0.05)。 结论 输卵管开窗术、输卵管切除术均可用于重度输卵管积水不孕症患者的输卵管疏通治疗,其中输卵管开窗术在保护卵巢储备功能方面似乎更具优势,但两种方式的IVF-ET成功率尚无明显差异。Objective : To investigate the effect of salpingotomy and salpingectomy on ovarian reserve function and the success rate of IVF-ET in patients with severe hydrosalpinx infertility. Methods : The data of 118 patients with severe hydrosalpinx infertility who underwent surgical treatment in our hospital from December 2015 to September 2018.The patients were divided into two groups according to different surgical methods:salpingotomy group( n =57)and salpingectomy group( n =61).The ovarian reserve function and the pregnancy outcome of IVF-ET were compared between the two groups. Results : The numbers of basal antral follicles and mature follicles on the HCG day in salpingotomy group were significantly more than those in salpingectomy group( P <0.05).There were no significant difference in ovarian volume and endometrial thickness on the day of transplantation between two groups( P > 0.05).There were no significant difference in serum FSH,LH and E 2 levels between two groups( P >0.05).There was no significant difference in implantation rate,clinical pregnancy rate and early abortion rate between the two groups( P >0.05). Conclusions : Both salpingotomy and salpingectomy can be used for treatment of fallopian tube dredging in patients with severe hydrosalpinx infertility.The salpingotomy has more advantages in protecting ovarian reserve function,but the success rate of IVF-ET is similar between the two surgical methods.
