摘 要:目的分析武汉市餐饮从业人员携带我国最常见沙门菌血清型菌株的脉冲场凝胶电泳指纹图谱及耐药谱,了解来自餐饮行业的沙门菌分离株流行病学特征。方法收集和鉴定武汉市2017年餐饮从业人员体检所检出的60株我国最常见5种沙门菌血清型(鼠伤寒、肠炎、德尔俾、阿贡纳及山夫登堡)菌株。在脉冲场凝胶电泳检测后,应用BioNumerics 7.6软件进行遗传图谱的聚类分析,并应用微量肉汤稀释法开展耐药谱检测。结果在PFGE聚类分析中,16株阿贡纳分为8个型别(相似度49.9%~100.0%);9株山夫登堡分为4个型别(相似度70.3%~93.3%)、9株鼠伤寒分为3个型别(相似度为81.0%~96.8%)、18株德尔俾仅分为2个型别(相似度80.6%~100.0%)、8株肠炎分为5个型别(相似度为65.6%~92.3%)。在耐药谱方面,哌拉西林、头孢唑啉和氨苄青霉素耐药率最高(8.33%),四环素、氨苄西林/舒巴坦居其次(5.00%)。在各型菌中,鼠伤寒耐药率最高(33.33%),其次是肠炎(25.00%),包括1株环丙沙星耐药的鼠伤寒沙门菌。耐药菌株总体占比达11.67%,其中多重耐药株达71.43%。结论武汉市人群携带的阿贡纳和肠炎沙门菌差异性均较大,而鼠伤寒和德尔俾沙门菌同源性较高。鼠伤寒和肠炎沙门菌的多重耐药率较高。根据耐药谱结果,哌拉西林、头孢唑啉、氨苄青霉素及四环素已不适于临床经验性用药。Objective To analyze the genetic fingerprint and antibiotics resistance of the most common Salmonella serotypes carried by catering employees in Wuhan,and to understand the epidemiology of Salmonella strains from catering industry.Methods In a 2017 routine health examination of catering employees in Wuhan,60 Salmonella strains were isolated and identified.These strains belong to the five most common Salmonella species in China,including S.typhimurium(18 strains),S.enteritidis(8 strains),S.derby(18 strains),S.argona(16 strains)and S.senftenberg(9 strains).In this study,the isolated strains were fingerprinted using pulsed-field gel electrophoresis,and then analyzed using BioNumerics 7.6 software.Sensitivity to various drugs was tested using microbroth dilution method.Results Cluster analysis showed that the 16 strains of Salmonella argona were divided into eight PFGE types with similarity range of 49.9%-100.0%;the 9 strains of Salmonella senftenberg were divided into four PFGE types with similarity range of 70.3%-93.3%;the 9 strains of Salmonella typhimurium were divided into three PFGE types with similarity range of 81.0%-96.8%;the 18 strains of Salmonella derby were only divided into two PFGE types with similarity range of 80.6%-100.0%,the 8 strains of Salmonella enteritidis were divided into five PFGE types with similarity rang of 65.6%-92.3%.Drug susceptibility test showed that the resistance rate of Salmonella to piperacillin,cefazolin and ampicillin was the highest(8.33%),and the resistance rate to tetracycline was the second highest(5.00%).Salmonella typhimurium has the highest resistance rate(33.33%),followed by Salmonella enteritidis(25.00%).A local strain of Salmonella typhimurium resistant to ciprofloxacin,a commonly used fluoroquinolone,was found.Among these strains,Salmonella-resistant strains accounted for 11.67%, and 71.43% of these strains were multi-drug resistant.ConclusionMolecular typing reveals that strains of Salmonella argina and Salmonella enteritidis carried by catering employees in Wuhan a
