摘　　要：目的 研究R院针对基于大数据的医疗服务评价改革的管理措施及结果,为政策施行及其他医疗机构的管理提供借鉴。方法 通过访谈了解R院针对改革的管理措施,通过收集R院及上海市市级医院2015年和2017年的医疗运营及病种数据,对比R院应对前后诊疗难度、医疗费用的变化。结果 R院通过规范药品及耗材使用,推进日间医疗及医联体建设等措施完善管理。相较于2015年,R院2017年药占比、耗占比低于市级医院平均水平,重点手术、重点病种、日间手术比例大幅上升,各项均次费用涨幅接近市级医院平均水平。结论 R院在提升卫生服务产出水平,合理规范医疗费用方面已初见成效。Objective: To study the management measures and results of medical service evaluation reform based on big data in R Hospital so as to provide reference for policy implementation and management in other medical institutions. Method: The authors through interviews,tried to understand the management measures of R Hospital in response to the reform by collecting medical operation and disease type data in R Hospital and Shanghai municipal hospitals in 2015 and 2017,and compared the difficulties of diagnosis and treatment,and the changes of medical cost before and after. Result: Through standardizing the use of drugs and consumables,R Hospital promotes the construction of daily medical treatment,medical consortium and other measures to improve management.Compared with 2015,the proportion of medicine and consumption in R Hospital in 2017 was lower than the average level of municipal hospitals,the proportion of key surgeries,key diseases and day surgeries increased significantly,and the average cost of each time increased by nearly the average level of municipal hospitals.The average cost,drug proportion and average daily hospitalization of R Hospital were lower than the average level of municipal hospitals. Conclusion: R Hospital has achieved initial results in improving health service output level and standardizing medical expenses.
