夜间急诊CT增强检查护理管理的改进措施与成效

摘　　要：目的探讨改进夜间急诊CT增强检查护理管理的措施与成效。方法选取我院2014年1月至2017年12月在我院晚上行急诊增强CT的631例患者,按时间段分成两组。2014年1月至2015年12月期间的286例为对照组,采用常规护理方法;2016年1月至2017年12月期间的345例为观察组,采用改进后的护理管理方法。其中男321例,女310例,年龄45~86岁,其中急性脑血管意外197例;主动脉夹层102例;动脉瘤89例;急腹症肠系膜上动静脉栓塞54例,其他189例。结果除5例患者外,其余患者均能顺利完成CT检查,其中27例患者出现轻中度过敏反应,观察组发生轻中度过敏的风险显著低于对照组(2. 32%VS 6. 64%,P<0. 008)。另外3例患者出现严重过敏,5例患者出现造影剂渗漏,因及时进行干预处理,均无出现术后皮肤坏死等严重并发症。结论科学护理法能显著降低急诊增强CT轻中度过敏的发生率,对提升医疗治疗、减少医疗纠纷具有重要的意义。Objective To explore the effect of improved nursing management on enhanced CT examination in night emergency.Methods From January 2014 to December 2017,631 patients with emergency enhanced CT in our hospital were divided into two groups according to the time period. 286 cases from January 2014 to December 2015 as control group were treated with routine nursing methods,while 345 cases from January 2016 to December 2017 as observation group were treated with improved nursing management method. Among them,321 cases were male and 310 cases were female,aged 45 to 86 years,including 197 cases of acute cerebrovascular accident,102 cases of aortic dissection,89 cases of aneurysm,54 cases of superior mesenteric arteriovenous embolism and189 cases of others. Results Except for 5 patients,the rest of the patients completed the CT examination,with 27 patients suffering from mild or moderate allergic reactions. The risk of mild or moderate allergic reactions in the observation group was significantly lower than that in the control group( 2. 32% VS 6. 64%,P<0. 008). In addition,3 patients had severe allergic reaction and 5 patients had contrast agent leakage. No postoperative skin necrosis or other serious complications occurred due to timely intervention. Conclusion Scientific nursing method can significantly reduce the incidence of mild or moderate allergy in patients who underwent enhanced CT in night emergency,which is of great significance for improving medical treatment and reducing medical disputes.
