射频消融房室结改良术中应用心房起搏的评价

摘　　要：为评价在射频消融房室结改良术中应用心房起搏的效果 ,对术中在心房起搏下放电的 34例进行了研究 ,并与以常规方法进行手术的另外 34例进行对照。结果显示 ,在心房起搏下放电能有效地抑制结性心律 ,放电中持续5s以上的结性心律发生率对照组为 94.1 % (32 / 34) ,而起搏组仅为 1 1 .8% (4/ 34) ,P <0 .0 0 1 ;减少总放电次数 ,对照组为 8.6± 5 .4次 ,起搏组为 3 .2± 1 .8次 ,P <0 .0 0 1 ;缩短总放电时间 (1 4 0 .6± 65 .4svs 1 86 .5± 82 .6s,P <0 .0 5) ;增加单次持续放电的成功率 (88.2 %vs 8.8% ,P <0 .0 0 1 ) ;同时可减少术中并发症及降低术后复发率 ,对照组有 4例术中出现一过性Ⅰ度房室阻滞 ,有 2例在术后 1年内复发 ,而起搏组无 1例发生上述情况。结论 :在房室结改良术中应用心房起搏的方法进行放电是安全的、有效的。To asses the effect of atrial pacing using in the procedure of radiofreque ncy atrioventricular node(AVN) modification.Thirty-four cases who received AVN modification under atrial pacing were compared with another 34 cases who receive d the procedure with routine method.The results demonstrated that the method wit h atrial pacing was effective in inhibiting the AVN rhythm appearance that was 94 .1% in control group,but only 11.8% in pacing group, P <0.001;reducing the tot al d ischarge time (140.6±65.4 vs 186.5±82.6 s P <0.05);increasing the success r ate by single discharge (88.2% vs 8.8% , P <0.001) and decreasing the compl icat ion and recurrence rate.There were 4 cases occurring Ⅰ degree atrioventricular block temporarily while discharge and 2 cases recurred during 1 year after the p rocedure in control group,but no one of those in pacing group.Conclusion:Atrial pacing using in the procedure of radiofrequency AVN modification was safety and effective.
